鼻腔扩容术治疗鼻阻塞69例临床分析

摘　　要：目的 探讨鼻腔扩容术治疗鼻腔结构异常导致的鼻阻塞的临床效果。方法 对九江市第一人民医院2019年6月至2020年10月收治的69例因鼻阻塞行鼻腔扩容术治疗患者的临床资料进行回顾性分析,其中单纯鼻中隔偏曲40例,单纯下鼻甲肥大5例,单纯中鼻甲肥大8例,单纯钩突肥大3例,混合型结构异常13例。根据患者病因不同制定个性化的鼻腔扩容术,包括鼻中隔矫正术、下鼻甲手术、中鼻甲手术等,术后6个月随访。比较患者术前术后鼻阻塞视觉模拟量表评分(VAS评分)及鼻阻力值(NAR值)改善情况。结果 69例患者均顺利完成手术,术前与术后VAS评分分别为(7.320±1.725)、(1.265±0.293)分,术前与术后NAR值分别为(0.415±0.029)、(0.158±0.019)kPa·L^(-1)·s^(-1),术后VAS评分、NAR值均较术前显著降低(P<0.001或P<0.01)。结论 通过对鼻阻塞患者的术前评估,制定个性化的鼻腔扩容术,可明显改善患者因鼻腔结构异常导致的鼻阻塞。Objective To investigate the clinical efficacy of nasal cavity dilatation in the treatment of nasal obstruction caused by structural abnormalities.Methods A retrospective analysis was conducted on 69 patients with nasal obstruction who underwent nasal cavity dilatation in Jiujiang First People’s Hospital from June 2019 to October 2020,including 40 cases of nasal septum deviation,5 cases of inferior turbinate hypertrophy,8 cases of middle turbinate hypertrophy,3 cases of uncinate process hypertrophy,and 13 cases of mixed structural abnormalities.Personalized nasal dilatation procedures were performed according to different etiologies,such as septum correction,inferior turbinate surgery and middle turbinate surgery.The patients were followed up 6 months after operation.The visual analogue scale(VAS)score and nasal airway resistance(NAR)value were measured before and after surgery.Results All the 69 patients completed the surgery successfully.The postoperative VAS score(1.265±0.293)was lower than the preoperative score(7.320±1.725)(P<0.001).In addition,the postoperative NAR decreased when compared to the preoperative value[(0.158±0.019)kPa·L^(-1)·s^(-1) vs(0.415±0.029)kPa·L^(-1)·s^(-1),P<0.01].Conclusion The development of personalized nasal cavity dilatation according to the preoperative evaluation can significantly improve nasal obstruction caused by nasal structural abnormalities.
