摘　　要：目的探讨Orem自理模式对改善乳腺癌术后老年患者自我管理和生活质量的影响。方法将86例乳腺癌手术老年患者随机分为对照组和观察组,对照组采用常规护理,观察组给予Orem自理模式干预。以自我管理能力、自我效能、生存质量为观察指标,比较两组的护理效果差异。结果实施Orem自理模式后,观察组患者的健康知识水平、自我概念、自我责任感、自我护理技能、自我护理能力总分较对照组显著更高(P<0.05);观察组患者在症状管理自我效能、疾病共性管理自我效能以及自我效能总分方面较对照组显著更高(P<0.05)。观察组患者生存质量各维度评分均较对照组显著更高(P<0.05)。结论Orem自理模式可显著提高乳腺癌手术老年患者的自我管理和生活质量。Objective To investigate the effect of Orem self-care model on improving self-management and quality of life of elderly patients after breast cancer surgery.Methods Eighty-six elderly patients with breast cancer were randomly divided into control group and observational group.The control group used conventional nursing,and observational group adopted Orem self-care model.Self-management ability,self-efficacy and quality of life were used as observation indicators to compare the differences in nursing effects between the two groups.Results After the implementation of Orem self-care model,the total scores of health knowledge,self-concept,self-responsibility,self-care skills and self-care ability in the observation group were significantly higher than those in the control group(P<0.05).The self-efficacy of symptom management,self-efficacy of disease commonality management and total self-efficacy scores in the observation group were significantly higher than those in the control group(P<0.05).Compared with the control group,the scores of quality of life in the observation group were higher(P<0.05).Conclusion Orem selfcare model can significantly improve self-management and quality of life in elderly patients undergoing breast cancer surgery.
