4种血液净化方式对维持性血液透析患者微炎症状态的影响

摘　　要：目的对比血液透析(hemodialysis,HD)、血液灌流(hemoperfusion,HP)、高通量透析(high-flow hemodialysis,HFD)、血液透析滤过(hemodiafiltration,HDF)4种血液净化方式对维持性血液透析患者微炎症状态的影响。方法选择中一东北国际医院于2018年1-6月收治120例尿毒症患者作为研究对象,所有研究对象均接受维持性血液透析,将其按照血液净化方式不同分为4组:HD组、HD+HP组、HFD组、HDF组,每组30例。对比4组患者治疗前后的微炎症状态。结果治疗前4组患者的C反应蛋白(CRP)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8),肿瘤坏死因子α(TNF-α)以及白蛋白(Ab)、血红蛋白(Hb)含量比较均无统计学差异,P>0.05;治疗后,HFD组、HDF组、HD+HP组患者的各项指标优于HD组,差异具有统计学意义,P<0.05,但HFD组、HDF组与HD+HP组之间各项指标并无统计学意义,P>0.05。结论HDF、HFD、HD+HP均能够有效清除维持性血液透析患者微炎症状态。Objective To compare the effects of hemodialysis(HD),hemoperfusion(HP),high-flow hemodialysis(HFD)and hemodiafiltration(HDF)on the micro inflammatory state of maintenance hemodialysis patients.Methods A total of 120 patients with uremia admitted to Northeast International Hospital from January to June 2018 were selected as the study objects.All the subjects received maintenance hemodialysis,and they were divided into four groups according to different blood purification methods:HD group,HD+HP group,HFD group,HDF group,30 cases in each group.The microinflammatory state of the four groups before and after treatment was compared.Results There was no significant difference in the contents of CRP,IL-6,IL-8,TNF-α,albumin(AB)and hemoglobin(HB)among the four groups before treatment(P>0.05).After treatment,the indexes of HFD group,HDF group and HD+Hp group were better than HD group,the difference was statistically significant(P<0.05).However,there was no significant difference among HFD group,HDF group and HD+Hp group(P>0.05).Conclusion HDF,HFD and HD+HP can effectively clear the microinflammatory state of maintenance hemodialysis patients.
