探讨夜磨牙症患者咬合及睡眠特征与弹性[牙合]垫的矫治作用

摘　　要：目的:观察弹性牙合垫对夜磨牙症患者咬合、睡眠情况的矫治效果,同时观察该症患者的咬合、睡眠特征。方法:选取2020年1月至2021年12月湖北省襄阳市谷城县人民医院口腔科收治的夜磨牙症患者144例作为研究对象,按照随机数字表法分为观察组和对照组,每组72例。观察组采取弹性牙合垫矫治,对照组给予常规治疗干预,观察2组治疗效果,同时比较2组患者睡眠特征的差异。结果:观察组治疗效果显著高于对照组,不良反应发生率显著低于对照组,观察组快动眼期、非快动眼期、微觉醒次数均高于对照组,RMMA次数低于对照组,观察组闭合时间、前伸牙合分离时间、左侧牙合分离时间、右侧牙合分离时间均高于对照组,2组比较差异均有统计学意义(P<0.05)。结论:夜磨牙症近年来发病率不断提高,大部分患者在患病后无法保障睡眠质量,严重者甚至出现呼吸暂停等症状,需要对患者进行牙合矫正,减少咬合问题,改善睡眠质量,弹性牙合垫的矫治作用显著,值得临床推广使用。Objective:To observe the effect of elastic occlusal pad on the occlusion and sleep of patients with nocturnal bruxism,and observe the characteristics of occlusion and sleep of patients with nocturnal bruxism.Methods:A total of 144 patients with nocturnal bruxism admitted to the Department of Stomatology of Gucheng County People’ s Hospital,Xiangyang City,Hubei Province from January 2020 to December 2021 were selected as the research objects.They were divided into observation group and control group according to the random number table methods,with 72 cases in each group.The observation group was treated with elastic occlusal pad,while the control group was treated with conventional treatment intervention.The therapeutic effects of 2 groups were observed,and the differences of sleep characteristics between the 2 groups were compared as well.Results:The therapeutic effect of the observation group was significantly higher than the control group,and the incidence of adverse reactions was significantly lower than the control group.The times of rapid eye movement,non rapid eye movement,and micro arousal in the observation group were higher than those of the control group,and the times of RMMA were lower than control group.The closing time,protrusive occlusal separation time,left occlusal separation time,and right occlusal separation time in the observation group were higher than those in the control group.The difference between the 2 groups was statistically significant(P<0.05).Conclusion:The incidence of nocturnal bruxism has been increasing in recent years.Most patients can not guarantee their sleep quality after the disease,and even have apnea and other symptoms in severe cases.It is necessary to correct the patient’s occlusion,reduce the occlusion problem,and improve their sleep quality.Elastic occlusal pads have a significant corrective effect,which is worthy of clinical promotion.
