摘 要:目的:探究新生儿脑病早期应用颅脑超声检查的应用价值。方法:选择本院2018年10月至2020年8月收治的经临床确诊为新生儿脑病患儿90例作为脑病组,并选取同期在本院行健康体检的新生儿88例作为对照组。所有参检新生儿均行颅脑超声检查,分析超声检查情况,另比较两组出生后24h内颅内血流情况,包括大脑中动脉(MCA)及前动脉(ACA)血流动力学指标[收缩期峰值流速(Vs)、舒张末期速度(Vd)、阻力指数(RI)]。结果:90例脑病组患儿中经颅脑超声检查发现异常62例(68.89%),其中轻度50例(80.65%),中重度12例(19.35%);脑病组新生儿MCA中Vs为(8.14±0.96)cm/s、Vd为(4.97±0.89)cm/s均低于对照组的(12.30±2.38)cm/s、(6.36±1.12)cm/.s,RI(0.77±0.12)高于对照组(0.60±0.09),差异有统计学意义(P<0.05);脑病组新生儿ACA中Vs为(31.92±3.55).cm/.s、Vd为(6.68±1.21)cm/s,均低于对照组的(42.56±4.40)cm/s、(12.56±2.69)cm/s,RI(0.82±0.14)高于对照组(0.61±0.08),差异有统计学意义(P<0.05)。结论:颅脑超声在新生儿脑病早期诊断中通过检测脑血流动力学状态判断病情,为临床诊疗提供可靠且客观的影像学及血流动力学参考依据,有助于及时明确病情严重程度并给予积极干预,具有较高的临床应用价值。Objective:To explore the application value of brain ultrasound in early neonatal encephalopathy.Methods:90 children with neonatal encephalopathy diagnosed clinically in our hospital from October 2018 to August 2020 were selected as the encephalopathy group,and 88 neonates who underwent physical examination in our hospital at the same time were selected as the control group.All the neonates involved in the examination were examined by brain ultrasound to analyze the ultrasonic examination.In addition,the intracranial blood flow within 24 hours after birth was compared between the two groups,including the middle cerebral artery(MCA)and anterior cerebral artery(ACA)hemodynamic indicators[peak systolic velocity(Vs),end diastolic velocity(Vd),resistance index(RI)].Results:Among the 90 children with encephalopathy,62 cases(68.89%)were found abnormal by brain ultrasound,of which 50 cases(80.65%)were mild,12 cases(19.35%)were moderate and severe;In the neonatal MCA of the encephalopathy group,the Vs and Vd of(8.14±0.96)cm/s and(4.97±0.89)cm/s were lower than those of the control group(12.30±2.38)cm/s and(6.36±1.12)cm/s,and the RI of(0.77±0.12)was higher than those of the control group(0.60±0.09),with statistical significance(P<0.05);In the neonatal ACA of the encephalopathy group,the Vs and Vd were(31.92±3.55)cm/s and(6.68±1.21)cm/s,which were lower than those of the control group(42.56±4.40)cm/s and(12.56±2.69)cm/s,and the RI was(0.82±0.14)higher than those of the control group(0.61±0.08),with statistical significance(P<0.05).Conclusion:In the early diagnosis of neonatal encephalopathy,brain ultrasound can judge the condition by detecting the cerebral hemodynamic state,provide reliable and objective imaging and hemodynamic reference for clinical diagnosis and treatment,help to determine the severity of the disease in time and give active intervention,which has higher clinical application value.
