摘　　要：目的分析经腹部联合经阴道超声诊断卵巢巧克力囊肿的临床价值。方法选取32例疑似卵巢巧克力囊肿患者,均接受经腹部超声、经阴道超声单独与联合检查,并以病理检查为金标准。对比三种检查方式与病理学检查的检出结果以及诊断效能,并分析其超声图像特征。结果32例疑似卵巢巧克力囊肿患者经病理检查,其中阳性21例,阴性11例;腹部超声检查,其中阳性11例,阴性21例,其中8例为真实卵巢巧克力囊肿患者;经阴道超声检查,其中阳性12例,阴性20例,其中10例为真实卵巢巧克力囊肿患者;经阴道联合腹部超声检查,其中阳性21例,阴性11例,其中20例为真实卵巢巧克力囊肿患者;经腹部超声、经阴道超声联合检查的敏感度、准确性以及阴性预测值均高于单项检查,漏诊率低于单项检查,差异有统计学意义(P<0.05)。结论相比于单项检查,经腹部联合经阴道超声检查能够有效提升卵巢巧克力囊肿的诊出率、敏感度、准确性以及阴性预测值,并降低漏诊风险,具有较高的诊断价值。Objective To analyze the diagnostic value of transabdominal combined transvaginal ultrasonography in the diagnosis of ovarian chocolate cyst.Methods 32 patients with suspected chocolate cysts were selected,and all received transabdominal ultrasonography,transvaginal ultrasonography alone and in combination,and pathological examination was the gold standard.The detection results and diagnostic efficiency of the three examination methods and pathological examinations were compared,and the characteristics of ultrasound images were analyzed.Results Pathological examination of 32 patients with suspected ovarian chocolate cysts,of which 21 cases were positive and 11 cases were negative;abdominal ultrasonography,11 cases were positive and 21 cases were negative,of which 8 cases were true ovarian chocolate cyst patients;transvaginal ultrasonography,12 cases were positive and 20 cases were negative,of which 10 cases were real ovarian chocolate cyst;transvaginal combined abdominal ultrasonography,21 cases were positive,11 cases were negative,of which 20 cases were real ovarian chocolate cyst.The sensitivity,accuracy and negative predictive value of combined transabdominal ultrasonography and transvaginal ultrasonography were higher than those of single examination,and the missed diagnosis rate was lower than that of single examination,the difference was statistically significant(P<0.05).Conclusion Compared with single examination,transabdominal combined transvaginal ultrasonography can effectively improve the diagnosis rate,sensitivity,accuracy and negative predictive value of ovarian chocolate cysts,and reduce the risk of missed diagnosis.It has high diagnostic value.
