孕中期血清sFlt-1、PLGF水平及子宫动脉搏动指数对子痫前期的预测价值分析

摘　　要：目的探索血清可溶性血管内皮生长因子受体-1(sFlt-1)、胎盘生长因子(PLGF)及两者的比值(sFlt-1/PLGF)与子宫动脉搏动指数(UAPI)对子痫前期(PE)临床预测诊断价值。方法选取71例PE患者为PE组,依据1∶2配对比例,随机选取同期142例健康孕妇为对照组。比较两组15~20周血清sFlt-1、PLGF、sFlt-1/PLGF水平及21~24周UAPI水平,并分析PLGF、sFlt-1、sFlt-1/PLGF、UAPI对PE的预测价值。结果与对照组比较,PE组UAPI、sFlt-1、sFlt-1/PLGF升高,PLGF降低;Logistic回归分析各指标对PE的预测ROC曲线:PLGF、sFlt-1、sFlt-1/PLGF、UAPI、UAPI+PLGF、UAPI+sFlt-1、UAPI+sFlt-1/PLGF的AUC值分别为0.719、0.743、0.769、0.759、0.784、0.796、0.791,差异有统计学意义(P<0.05)。结论中孕期血清PLGF、sFlt-1、sFlt-1/PLGF及UAPI对于PE均有一定的预测效能,且多指标联合诊断预测效能大于单一指标。Objective To explore clinical predictive diagnostic value of serum soluble vascular endothelial growth factor receptor-1(sFlt-1),placental growth factor(PLGF)and their ratio(sFlt-1/PLGF)and uterine artery pulsatility index(UAPI)on preeclampsia(PE).Methods 71 PE patients were selected as the PE group,and 142 healthy pregnant women were randomly selected as the control group according to the matching ratio of 1:2.The serum sFlt-1,PLGF,sFlt-1/PLGF levels in the two groups at 15-20 weeks were compared,and the UAPI levels in the two groups at 21-24 weeks were compared,and the predictive diagnostic value of PLGF,sFlt-1,sFlt-1/PLGF,and UAPI in PE was analyzed.Results Compared with the control group,UAPI,sFlt-1,s Flt-1/PLGF increased and PLGF decreased in PE group;Logistic regression analysis of the predictive ROC curve of each index for PE:the AUC values of PLGF,sFlt-1,sFlt-1/PLGF,UAPI,UAPI+PLGF,UAPI+sFlt-1,UAPI+sFlt-1/PLGF were 0.719,0.743,0.769,0.759,0.784,0.796,0.791,the difference was statistically significant(P<0.05).Conclusions Serum PLGF,sFlt-1,sFlt-1/PLGF and UAPI in the second trimester have predictive efficacy for PE,and the predictive efficacy of multi-index combined diagnosis is greater than that of a single index.
