胰岛素治疗妊娠糖尿病患者起始孕周与妊娠结局的相关性分析

摘　　要：目的 探讨胰岛素治疗妊娠糖尿病患者起始孕周与妊娠结局的相关性。方法 选取90例妊娠糖尿病患者,以胰岛素治疗起始孕周为依据分组,早期组(45例)为治疗起始孕周≤32周,晚期组(45例)为治疗起始孕周>32周,两组均采取胰岛素治疗,分析治疗后两组患者血糖和妊娠结局的差异。结果 与治疗前比较,治疗后两组的空腹血糖和餐后2 h血糖水平均降低,差异具有统计学意义(P<0.05);但治疗前和治疗后两组组间进行比较,差异均无统计学意义(P>0.05)。早期组胎儿窘迫和巨大儿的发生率分别为2.22%、11.11%,均低于晚期组的15.56%、40.00%,差异有统计学意义(P<0.05);而两组剖宫产、早产、妊娠高血压及死胎的发生率进行比较,差异均无统计学意义(P>0.05)。结论 妊娠糖尿病临床治疗过程中,需要对患者日常饮食合理控制,及早采用胰岛素治疗有助于改善妊娠结局。Objective To investigate the relationship between the initial gestational weeks and pregnancy outcomes in patients with gestational diabetes treated with insulin.Methods 90 patients with gestational diabetes mellitus were selected.They were divided into two groups according to the initial gestational weeks of insulin therapy.The early group(45 cases) was treated with the initial gestational weeks ≤32 weeks,and the late group(45 cases) was treated with the initial gestational weeks >32 weeks.Both groups were treated with insulin,and the differences in blood glucose and pregnancy outcomes between the two groups after treatment were analyzed.Results Compared with before treatment,the fasting blood glucose and 2 h postprandial blood glucose levels of the two groups decreased after treatment,and the difference was statistically significant(P0.05).The incidences of fetal distress and macrosomia in the early group were 2.22% and 11.11%,which were lower than 15.56% and 40.00% in the late group,and the difference was statistically significant(P0.05).Conclusion In the course of clinical treatment of gestational diabetes,it is necessary to control the patients’ daily diet reasonably.Early insulin treatment can improve the pregnancy outcome.
