输卵管妊娠患者行腹腔镜下输卵管妊娠物清除联合近端阻断术治疗的前瞻性随机研究

摘　　要：目的 探讨输卵管妊娠患者行腹腔镜下输卵管妊娠物清除联合近端阻断术治疗对于妊娠结局的影响。方法 选取66例输卵管妊娠患者,将研究对象使用随机数字表法分为研究组和对照组,各33例。对照组患者行腹腔镜下输卵中妊娠物清除术,研究组患者在对照组基础上进行近端阻断术,两组患者均持续随访12个月。比较两组患者手术相关指标,术后1、12个月性激素,术后12个月内妊娠情况。结果 两组患者各项手术相关指标、术后1、12个月血清卵泡刺激素(FSH)、黄体生成素(LH)水平相比,差异无统计学意义(P>0.05);研究组术后12个月宫内妊娠率高于对照组,异位妊娠率低于对照组,研究组未受孕率比对照组更低,差异有统计学意义(P<0.05)。结论 输卵管妊娠患者行腹腔镜下输卵管妊娠物清除联合近端阻断术治疗对其卵巢储备功能无较大影响,并有效提高宫内妊娠率。Objective To investigate the effect of laparoscopic tubal pregnancy clearance combined with proximal occlusion on pregnancy outcome in patients with tubal pregnancy.Methods A total of 66 patients with tubal pregnancy were selected,and the subjects were divided into study group and control group by random number table method,with 33 cases in each.The patients in the control group underwent laparoscopic removal of pregnant objects in the fallopian,and the patients in the study group underwent proximal occlusion on the basis of the control group.The patients in both groups were followed up for 12 months.The operation related indexes,sex hormone at 1 and 12 months after operation and the outcome of second pregnancy within 12 months after operation were compared between the two groups.Results There was no significant difference between the two groups in various operation related indexes and the levels of serum follicle stimulating hormone(FSH),luteinizing Hormone(LH) at 1 and 12months after operation(P>0.05);The intrauterine pregnancy rate 12 months after operation in the study group was higher than that in the control group;the ectopic pregnancy rate was lower than that in the control group;the non pregnancy rate in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Patients with tubal pregnancy who underwent laparoscopic tubal pregnancy clearance combined with proximal occlusion had no significant effect on their ovarian reserve,effectively improved the intrauterine pregnancy rate and reduced adverse pregnancy outcomes.
