摘 要:目的 分析输卵管妊娠的超声分型及临床意义。方法 选择12例腹腔镜手术明确为输卵管妊娠的患者,使用彩色多普勒超声仪检查患者有无盆腔积液、积液深度,异位妊娠方位及子宫内膜有无病理等情况。结果 12例均为输卵管妊娠患者,其中有盆腔积液者8例,占66%,积液最多深度约34 mm。左侧输卵管妊娠6例(50%),右侧输卵管妊娠6例(50%)。壶腹部妊娠11例,占92%,伞部妊娠1例。超声分型“面包圈”孕囊型6例,其中囊内可见卵黄囊者3例,混合性包块型5例,破裂型1例。12例输卵管妊娠患者均经腹腔镜手术证实,手术中均可见绒毛。11例手术时未见破口,1例可见破口。9例子宫内膜病理为分泌期子宫内膜局部腺体呈A-S样反应、间质蜕膜变。结论 超早期诊断输卵管妊娠是临床保守治疗、保留输卵管功能的前提。动态超声分型(阴道超声为主)及诊断性刮宫,可改善输卵管妊娠患者预后。Objective To analysize the ultrasound classification and clinical significance of tubal pregnancy.Methods A total of 12 patients with tubal pregnancy confirmed by laparoscopic surgery were selected,and color Doppler ultrasonography was used to check whether the patients had pelvic effusion,the depth of effusion,the position and condition of ectopic pregnancy,and whether there was pathology in the endometrium.Results The 12 cases were all tubal pregnancy patients,of which 8 cases had pelvic effusion,accounting for 66%,and the maximum depth of effusion was about 34 mm.There were 6 cases(50%) of left tubal pregnancy and 6 cases(50%) of right tubal pregnancy.11 cases of ampulla pregnancy,accounting for 92%,1 case of umbrella pregnancy.Ultrasound classification included 6 cases of "doughnut" gestational sac type,including 3 cases of yolk sac in the cyst,5 cases of mixed mass type,and 1 case of rupture type.All 12 patients with tubal pregnancy were confirmed by laparoscopic surgery,and villi were visible during the operation.In 11 cases,no rupture was found during the operation,and 1 case had a rupture.Endometrial pathology in 9 cases was A-S-like reaction of local glands in secretory stage and interstitial decidualization.Conclusion Ultra-early diagnosis of tubal pregnancy is the premise of conservative clinical treatment and preservation of tubal function.Dynamic ultrasound classification(mainly vaginal ultrasound) and diagnostic curettage can improve the prognosis of patients with tubal pregnancy.
