腹部超声检查与阴道彩色多普勒超声检查对妇科急腹症的诊断效能比较

摘　　要：目的比较腹部超声检查与阴道彩色多普勒超声(阴道彩超)检查诊断妇科急腹症的准确率。方法随机筛选经手术病理诊断为妇科急腹症的患者35例,均采用腹部超声检查和阴道彩超检查。比较两种检查方式的准确率。结果35例妇科急腹症患者的病理结果为胚胎停育8例,不完全流产6例,宫腔粘连12例,肿瘤破裂4例,急性盆腔炎5例;阴道彩超检查的诊断结果为胚胎停育8例,不完全流产5例,宫腔粘连12例,肿瘤破裂4例,急性盆腔炎4例,诊断准确率为94.3%(33/35),误诊率为5.7%(2/35);腹部超声检查的诊断结果为胚胎停育7例,不完全流产4例,宫腔粘连10例,肿瘤破裂3例,急性盆腔炎3例,诊断准确率为77.1%(27/35),误诊率为22.9%(8/35)。阴道彩超检查的诊断准确率高于腹部超声检查,误诊率低于腹部超声检查,差异有统计学意义(P<0.05)。结论阴道彩超检查诊断妇科急腹症的准确率高于腹部超声检查,但仍建议临床上可联合应用两种诊断方式诊断妇科急腹症,以便进一步提高诊断准确率,使患者尽早接受全面而有效的治疗。Objective To compare accuracy rate of abdominal ultrasound examination and vaginal color Doppler ultrasound examination in the diagnosis of gynecological acute abdomen.Methods Thirty-five patients with gynecological acute abdomen diagnosed by operative pathology were randomly selected,and all of them were conducted with abdominal ultrasound examination and vaginal color Doppler ultrasound examination.The accuracy rate was compared between two examination methods.Results Pathological results of 35 cases with gynecological acute abdomen were as follows:8 cases of embryo suspension,6 cases of incomplete abortion,12 cases of intrauterine adhesion,4 cases of tumor rupture and 5 cases of acute pelvic inflammation.The diagnostic results of transvaginal color Doppler ultrasound examination were as follows:8cases of embryo suspension,5 cases of incomplete abortion,12 cases of intrauterine adhesion,4 cases of tumor rupture and 4 cases of acute pelvic inflammation,the diagnostic accuracy rate was 94.3%(33/35),and the misdiagnosis rate was 5.7%(2/35).The diagnostic results of abdominal ultrasound examination were as follows:7 cases of embryo abortion,4 cases of incomplete abortion,10 cases of intrauterine adhesion,3 cases of tumor rupture and 3 cases of acute pelvic inflammation,the diagnostic accuracy rate was 77.1%(27/35),and the misdiagnosis rate was 22.9%(8/35).The diagnostic accuracy rate of vaginal color Doppler ultrasound examination was significantly higher than that of abdominal ultrasound examination,while the misdiagnosis rate was significantly lower than that of abdominal ultrasound examination(P<0.05).Conclusion The accuracy rate of vaginal color Doppler ultrasound examination is higher than that of abdominal ultrasound examination in the diagnosis of gynecological acute abdomen,but it is still suggested that the two diagnostic methods should be used in combination in clinical diagnosis of gynecological acute abdomen,so as to further increase the diagnostic accuracy rate and enable the patients to receive compre
