基于HIS系统的手术患者预住院解决方案

摘　　要：目的为了缓解目前大型三甲医院床位紧张、患者住院困难等现状,我院综合床位有限与医院信息化发展情况,提出了适合本院的预住院管理模式。方法根据《广州市社会医疗保险医疗费用结算办法》(穗医保规字〔2019〕10号)政策指引,针对本地参保、自费且病情相对稳定可择期手术治疗的患者,我院制定预住院管理模式,并成立了预住院管理中心。预住院患者必须排除内科疾患且符合手术指征,则可在门诊完成正式住院前(10天内)一切必要的辅助检查,患者的检查结果符合入院条件且临床有空闲床位,即可正式住院。患者的住院日从正式住院开始计算,住院后费用仅限于广州市医保结算;此外,条件不符合的患者自费办理出院。我院在现有的医院信息管理系统(HIS系统)的基础上,改进了患者住院流程,最大程度地保持了医护人员原有的操作习惯,同时满足了患者对于住院就诊治疗的迫切需求。结果提高了患者的就医满意度和医疗服务效率,缩短了患者平均住院日、缓解了床位紧张以及减少了病人的流失。Objective To alleviate the current situations of large general hospitals,including limited inpatient beds and difficulty in hospitalization,considering the needs of inpatient beds and the development status of hospital information,our hospital proposes a pre-hospitalization management model suitable for the current situations of our hospital.Methods In accordance with the policy guidance of Guangzhou Social Medical Insurance Medical Expense Settlement Method(SHBGZ[2019]No.10),our hospital has developed a pre-hospitalization management model and established a pre-hospitalization management center for local patients who are insured,pay their own expenses,and have a relatively stable condition and can receive elective surgical treatment.Pre-hospital patients,who meet the surgical indications and exclude internal disease,will complete all necessary auxiliary examinations before formal hospitalization(within 10 days)in outpatient departments.If the examination results meet the admitted conditions and inpatient bed is available in the clinical department,we will transfer the patients into formal hospitalization.The hospital stay of the patient is counted from the formal hospitalization.After the formal hospitalization,the cost is charged based on medical insurance(only Guangzhou medical insurance),and those who cannot be operated surgery are discharged at their own expense.On the basis of the existing hospital information system(HIS),we reform the pre-hospitalization process to keep the original operating habits of medical staff unchanged with greatest extent and to meet the urgent needs of surgical patients at the same time.Results It improves the medical satisfaction of patients and the efficiency of medical services.It also shortens the average hospital stay of patients,alleviates situation of limited inpatient beds,and reduces the loss of patients.
