摘 要:目的观察多重荧光PCR法和免疫荧光法在辅助诊断急性呼吸道感染(acute respiratory infection,ARI)中的一致性。方法对2021年12月至2022年4月山东阳光融和医院620例急性呼吸道感染住院病例采集咽拭子标本,分别采用多重荧光PCR法和免疫荧光法检测6种呼吸道感染病原体(甲型流感病毒、乙型流感病毒、呼吸道合胞病毒、腺病毒、副流感病毒Ⅰ型及副流感病毒Ⅲ型),以评估两种方法在诊断呼吸道病毒感染中的一致性、灵敏性、特异性。结果620例ARI标本中,多重荧光PCR法检测阳性率高于免疫荧光法,差异具有统计学意义(χ^(2)=26.28,P<0.005)。两种方法显示乙型流感病毒、呼吸道合胞病毒检出率最高,其次是腺病毒、副流感病毒Ⅲ型。结论多重荧光PCR法检测6种呼吸道病毒阳性率显著高于免疫荧光法,且与临床表现更相符,提示其对于呼吸道病毒检测有更好的临床应用价值。Objective To observe the consistency of multiple real-time quantitative PCR and immunofluorescence method in the auxiliary diagnosis of acute respiratory tract infection(ARI).Methods We collected throat swabs from 620 acute respiratory tract infection cases in Shandong Sunshine Union Hospital between December,2021 and April,2022,and used RT-PCR and IF to detect 6 respiratory tract infection pathogens(Influenza A,Influenza B,Respiratory Syncytial Virus,Adenovirus,Parainfluenza Ⅰ and Parainfluenza Ⅲ),to evaluate the consistency,sensitivity and specificity of the two methods in diagnosing respiratory viral infections.Results Among 620 ARI specimens,the positive rate of multiple real-time quantitative PCR was significantly higher than that of immunofluorescence method(χ^(2)=26.28,P<0.005).Two methods showed that influenza B virus and respiratory syncytial virus had the highest detection rate,followed by adenovirus and parainfluenza virus type 3.Conclusion The multiplex RT-PCR method detects 6 respiratory viruses with a higher positive rate than the immunofluorescence method,and is more consistent with clinical manifestations,suggesting that it has a better detection performance of respiratory viruses.
