摘 要:目的评价荧光染色法在真菌镜检中的应用价值。方法收集216例临床标本,同时进行革兰染色镜检、荧光染色镜检和真菌培养,3种方法中任意一种为阳性认为该标本真菌检测阳性,并以此标准,计算3种方法的真菌检出率,革兰染色法和荧光染色法的灵敏度和阴性预测值的差异,对比革兰染色与荧光染色镜下特点。结果216例临床标本,革兰染色法、荧光染色法和真菌培养法的真菌检出率分别为11.57%、37.50%和42.13%,革兰染色法和荧光染色法灵敏度分别是23.15%和75.00%,阴性预测值分别是56.54%和80.00%,革兰染色法与荧光染色法或真菌培养法比较差异均有统计学意义(P<0.05)。荧光染色法与真菌培养法比较差异无统计学意义(P>0.05)。荧光染色法比革兰染色法镜下真菌形态更清晰易辨。结论荧光染色法是快捷、准确的真菌检测方法,值得临床推广应用。Objective To evaluate the value of fluorescence staining in microscopic examination of fungi.Methods A total of 216 clinical samples were collected for the study.Each sample was subjected to three methods(Gram staining,fluorescence staining microscopy examination and fungal culture)for the detection.The fungi detection was defined as positive if the results in any of these three methods presented positive.With this criterion,the fungal detection rates of the three methods were calculated,as well as the differences of sensitivity and negative predictive value between Gram staining and fluorescence staining.Moreover,the images under Gram staining and fluorescence staining were also compared.Results In 216 clinical samples,the fungal detection rates of Gram staining,fluorescence staining and fungal culture were 11.57%,37.50%and 42.13%,respectively.The sensitivity of Gram staining and fluorescence staining were 23.15%and 75.00%,respectively,and the negative predictive value were 56.54%and 80.00%,respectively.There were significant differences between Gram staining and fluorescent staining or fungal culture method(P<0.05),while there was no significant differences between fluorescent staining and fungal culture method(P>0.05).Compared with Gram staining,the fungal structures under fluorescence staining showed a better quality and were easier to be distinguished.Conclusion Fluorescence staining is a fast and accurate method for the detection of fungal organisms,which is worthy of popularizing in clinical applications.
