摘　　要：目的分析基于系统性营养评估的支持干预在慢性心力衰竭合并营养不良患者中的应用价值。方法抽取2021年1月至2022年3月河南省人民医院收治的慢性心力衰竭合并营养不良患者82例,采用随机数字表法将其分为研究组与对照组,每组41例。对照组采用常规营养支持方案,研究组采用基于系统性营养评估的支持干预。比较两组干预前后的营养状况(血红蛋白、前白蛋白、总白蛋白)、心功能[左心室射血分数(LVEF)、左室心排血量(LVCO)、心脏指数(CI)]及免疫功能[免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)]。结果干预后,两组血红蛋白、前白蛋白、总白蛋白、LVEF、LVCO、CI和IgA、IgG、IgM均较干预前增高,且研究组高于对照组(P<0.05)。结论慢性心力衰竭合并营养不良患者采用基于系统性营养评估的支持干预,可改善患者的营养状况,提高免疫功能和心功能。Objective To analyze the application value of supportive intervention based on systematic nutritional assessment for patients with chronic heart failure complicated by malnutrition.Methods A total of 82 patients with chronic heart failure complicated by malnutrition treated in Henan Provincial People's Hospital from January 2021 to March 2022 were selected and divided into study group and control group by random number table method,with 41 cases in each group.The control group received routine nutritional support programs,and the study group received supportive intervention based on systematic nutritional assessment.Nutritional status(hemoglobin,prealbumin,total albumin),cardiac function assessed by left ventricular ejection fraction(LVEF),left ventricular cardiac output(LVCO),cardiac index(CI),immune function assessed by immunoglobulin A(IgA),immunoglobulin G(IgG),immunoglobulin M(IgM)before and after intervention were compared between the two groups.Results Hemoglobin,prealbumin,total albumin,LVEF,LVCO,CI,IgA,IgG,and IgM in the two groups after intervention were all higher than those before intervention,and those in the study group were higher than those in the control group(P<0.05).Conclusions In patients with chronic heart failure and malnutrition,supportive intervention based on systematic nutritional assessment can improve nutritional status,immune function and cardiac function.
