摘　　要：目的探讨支原体肺炎(MPP)患儿辅助性T细胞17(Th17)/调节性T细胞(Treg)和相关细胞因子的表达及与病情的相关性。方法前瞻性选取2017年6月至2019年9月在唐山市妇幼保健院诊治的MPP患儿80例,根据患儿MP IgM抗体滴度分为低滴度组(1∶40~1∶320)45例和高滴度组(≥1∶640)35例。选取同期体检健康儿童30名作为对照组。流式细胞法检测外周血Th17/Treg及相关细胞因子水平。根据临床肺部感染评分(CPIS)将80例MPP患儿分为低危组(22例)、中危组(40例)、高危组(18例)。比较低滴度组、高滴度组、对照组外周血Th17、Treg细胞比率、Th17/Treg比值以及Th17、Treg相关细胞因子水平;比较不同CPIS评分MPP患儿Th17、Treg及Th17/Treg水平;采用Pearson相关分析MPP患儿外周血Th17、Treg细胞比率及Th17/Treg比值水平与病情的相关性。结果高滴度组和低滴度组外周血Th17和Th17/Treg水平均高于对照组,外周血Treg水平均低于对照组,且高滴度组以上指标均较低滴度组变化更显著,差异均有统计学意义(P<0.05)。高滴度组和低滴度组外周血IL-17和TNF-α水平均高于对照组,外周血IL-10和TGF-β水平均低于对照组,且高滴度组以上指标均较低滴度组变化更显著,差异均有统计学意义(P<0.05)。高危组和中危组外周血Th17、Th17/Treg水平均高于低危组,外周血Treg水平均低于低危组,且高危组以上指标较中危组更显著,差异均有统计学意义(P<0.05)。Pearson相关分析显示,MPP患儿外周血Th17及Th17/Treg水平均与CPIS评分呈正相关(r=0.699、0.862,P<0.001)。Treg与CPIS评分呈负相关(r=-0.749,P<0.001)。结论MPP患儿MP IgM抗体滴度越高,外周血Th17、Th17/Treg、IL-17和TNF-α水平则越高,Treg、IL-10和TGF-β越低。外周血Th17、Treg和Th17/Treg水平与病情严重程度密切相关,可以作为评估MPP病情的检测指标。Objective To investigate the expression characteristics of helper T cell 17(Th17)/regulatory T cell(Treg)and related cytokines in children with mycoplasma pneumoniae pneumonia(MPP)and their correlation with the disease.Methods From June 2017 to September 2019,80 children with MPP diagnosed in Maternal and Child Health Care Hospital of Tangshan City were prospectively selected,which was divided into two subgroups according to the titer of MP IgM antibody,the low titer group(1∶40~1∶320)(n=45)and the high titer group(≥1∶640)(n=35).Thirty children with physical examination in the same period were selected as the control group.The levels of Th17/Treg and related cytokines in each group were detected by flow cytometry.According to the clinical pulmonary infection score(CPIS),80 MPP children in bacterial group were divided into three subgroups:the low risk group(n=22),the medium risk group(n=40)and the high risk group(n=18).The ratio of Th17 and Treg cells in peripheral blood,Th17/Treg ratio and the levels of Th17 and Treg-related cytokines in the low titer group,high titer group and control group were compared;the levels of Th17,Treg and Th17/Treg in children with MPP with different CPIS scores were compared.The correlation between the peripheral blood Th17,Treg cell ratio and Th17/Treg ratio in children with MPP and the disease was analyzed using Pearson correlation.Results The levels of Th17 and Th17/Treg in peripheral blood of the high titer and low titer groups were higher than those in the control group,the level of Treg in peripheral blood of the high titer group and the low titer group was lower than that in the control group,and the above indicators in the high titer group were all changed more significantly in the lower titer group,the differences were statistically significant(P<0.05).The levels of IL-17 and TNF-αin peripheral blood of the high titer group and the low titer group were higher than those of the control group,the levels of IL-10 and TGF-βin peripheral blood of the high titer group and
