血清PON-1、LOOH和LIVIN在中耳胆脂瘤患者血清中的表达意义及其与患者病情严重程度的关系

摘　　要：目的探讨与分析血清对氧磷酯酶-1(PON-1)、脂氢过氧化物(LOOH)和凋亡蛋白抑制因子LIVIN在中耳胆脂瘤患者血清中的表达意义及与患者病情严重程度的关系。方法采用回顾性研究,选择2018年3月到2022年3月在武威市人民医院诊治的中耳胆脂瘤患者68例作为研究对象。所有患者均给予手术治疗,对听小骨破坏程度分级进行判定,Ⅰ级24例,Ⅱ级26例,Ⅲ级10例,Ⅳ级8例。检测所有患者血清PON-1、LOOH和LIVIN水平,分析其与听小骨破坏程度分级的相关性,并采用Logistic回归分析影响中耳胆脂瘤患者听小骨破坏程度分级的因素。结果在68例患者中,不同听小骨破坏程度分级患者的血清PON-1、LOOH和LIVIN含量比较,差异均有统计学意义(P<0.05),分级越高的患者,血清PON-1、LOOH和LIVIN含量越高。Spearsman分析显示中耳胆脂瘤患者的血清PON-1、LOOH和LIVIN含量与听小骨破坏程度分级呈显著正相关(r=0.672、0.597、0.733,P<0.05)。Logistic回归分析显示血清PON-1、LOOH和LIVIN含量为影响中耳胆脂瘤患者听小骨破坏程度分级的独立危险因素(OR=2.524,95%CI=1.183~5.663;OR=2.457,95%CI=1.111~5.436;OR=2.473,95%CI=1.109~5.148,P<0.05)。结论不同病情严重程度的中耳胆脂瘤患者血清PON-1、LOOH和LIVIN表达水平存在差异,血清PON-1、LOOH和LIVIN含量与听小骨破坏程度分级存在相关性。Objective To explore and analysis the expressions of serum PON-1,LOOH and LIVIN in serum of patients with middle ear cholesteatoma and the severity of the patient's condition.Methods From March 2018 to March 2022,68 cases of patients with middle ear cholesteatoma diagnosed and treated in People's Hospital of Wuwei City were retrospectively selected as the research subjects.All patients were given surgical treatment,and the degree of auditory ossicle destruction was graded.There were 24 cases of grade I,26 cases of grade,10 cases of gradeⅢ,and 8 cases of gradeⅣ.Serum levels of PON-1,LOOH and LIVIN were detected in all patients,and their correlation with the ossicle destruction grade was analyzed.Logistic regression was used to analyze the factors affecting the ossicle destruction grade in patients with middle ear cholesteatoma.Results There were significant differences in serum PON-1,LOOH and LIVIN levels in patients compared with different ossicle damage grades(P<0.05),with the higher the grades,with the higher the serum PON-1,LOOH and LIVIN levels.Spearsman analysis showed that the serum levels of PON-1,LOOH and LIVIN in patients with middle ear cholesteatoma were significantly positively correlated with the grade of ossicle destruction(r=0.672,0.597,0.733,P<0.05).Logistic regression analysis showed that serum levels of PON-1,LOOH and LIVIN were independent risk factors for the grading of auditory ossicle destruction in patients with middle ear cholesteatoma(OR=2.524,95%CI=1.183-5.663;OR=2.457,95%CI=1.111-5.436;OR=2.473,95%CI=1.109-5.148,P<0.05).Conclusion There are differences in the expression levels of serum PON-1,LOOH and LIVIN in patients with middle ear cholesteatoma of different severity,and there are correlation between the levels of serum PON-1,LOOH and LIVIN and the degree of ossicle destruction.
