摘  要:目的分析术前多频稳态残余听力对人工耳蜗术后言语和听觉功能恢复的影响。方法抽取2017年5月至2021年5月郑州市第三人民医院收治的78例感音神经性耳聋拟行人工耳蜗术且术前存在残余低频或高频残余听力的患儿作为研究对象,按照术前多频稳态诱发电位检查结果,将38例0.5、1.0 kHz作为低频组,将40例2.0、4.0 kHz作为高频组。所有患儿人工耳蜗术后1个月开机后实施系统化言语训练。比较两组患儿干预6个月后言语识别率;干预3、6、12个月后,比较两组患儿言语可懂度(SIR)评分、听觉行为分级问卷(CAP)评分、听觉整合量表(MAIS)评分。结果干预6个月后,高频组单音节词、双音节词、短句识别率(70.00%,28/40;67.50%,27/40;75.00%,30/40)均高于低频组(45.00%,18/38;42.50%,17/38;50.00%,20/38),P均<0.05。干预3、6、12个月,高频组SIR、CAP、MAIS评分均高于低频组(P均<0.05)。结论与术前低频残余听力患儿相比,高频残余听力更有利于人工耳蜗术后言语和听觉功能恢复。Objective To analyze the effect of preoperative multifrequency steady-state residual hearing on recovery of speen and auditory function after cochlear implantation.Methods A total of 78 children with sensorineural deafness who planned to have cochlear implantation and had residual low-frequency or high-frequency residual hearing before operation in the Zhengzhou Third People’s Hospital from May 2017 to May 2021 were selected as the research objects.According to the preoperative multifrequency steady-state evoked potential test results,38 cases with 0.5 kHz and 1.0 kHz were selected as the low frequency group,and 40 cases with 2 kHz and 4 kHz were selected as the high frequency group.All children received systematic speech training one month after cochlear implantation.The speech recognition rates of the two groups were compared after 6 months of intervention.After 3,6 and 12 months of intervention,The speech intelligibility rating(SIR)score,categories of audtory performance(CAP)score,meaningful auditory integration scale(MAIS)score of the children were compared between the two groups.Results After 6 months of intervention,the recognition rates of monosyllabic words,disyllabic words and short sentences in the high frequency group were higher than those in the low frequency group(P<0.05).After 3 months,6 months and 12 months of intervention,the scores of SIR,CAP and MAIS in the high frequency group were higher than those in the low frequency group(P<0.05).Conclusions Compared with preoperative low-frequency residual hearing,high-frequency residual hearing is more conducive to the recovery of speech and auditory function after cochlear implantation.
