托法替布联合艾拉莫德治疗类风湿关节炎的效果与安全性

摘　　要：目的探讨托法替布联合艾拉莫德治疗类风湿关节炎的效果与安全性。方法抽取2020年1月至2021年8月焦作市人民医院收治的类风湿关节炎患者102例,以随机数字表法分为研究组与对照组,每组51例。对照组采用艾拉莫德治疗,研究组采用艾拉莫德加用托法替布治疗。比较两组疗效、关节肿胀指数(ASI)评分、关节疼痛指数(API)评分、红细胞沉降率(ESR)、C反应蛋白(CRP)、类风湿因子(RF)、E-选择素、P-选择素、Β-胶原降解产物(Β-CTX)、总Ⅰ型胶原氨基端延长肽(T-PIMP)、骨钙素(BGP)及不良反应发生率。结果研究组总有效率(96.08%,49/51)高于对照组(74.51%,38/51),P<0.05。治疗后,研究组ASI、API评分均低于对照组(P均<0.05);ESR、CRP、RF、E-选择素、P-选择素、Β-CTX水平低于对照组,T-PIMP、BGP高于对照组(P<0.05)。两组不良反应发生率比较差异未见统计学意义(P>0.05)。结论托法替布联合艾拉莫德治疗类风湿关节炎可增强骨代谢,降低E-选择素和P-选择素含量,减轻炎性刺激,改善关节功能状态,临床效果显著。Objective To analyze the efficacy and safety of tofacitinib combined with iguratimod in the treatment of rheumatoid arthritis.Methods A total of 102 patients with rheumatoid arthritis treated in Jiaozuo People’s Hospital from January 2020 to August 2021 were selected,and they were divided into study group and control group by random number table method,with 51 case in each group.The control group was treated with iguratimod,and the study group was treated with iguratimod combined with tofacitinib.The curative effect,joint swelling index(ASI)score,joint pain index(API)score,erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),rheumatoid factor(RF),E-selectin,P-selectin B-collagen degradation products(Β-CTX),total type I collagen amino terminal lengthening peptide(T-PIMP),osteocalcin(BGP),and the incidence of adverse reactions were compared between the two groups.Results The total effective rate of the study group(96.08%,49/51)was higher than that of the control group(74.51%,38/51),P<0.05.After treatment,the ASI and API scores of the study group were lower than those of the control group(all P<0.05).After treatment,the levels of ESR,CRP,RF,E-selectin,P-selectin andΒ-CTX in the study group were lower than those in the control group,while the levels of T-PIMP and BGP were higher than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions Tofacitinib combined with iguratimod in the treatment of rheumatoid arthritis can enhance bone metabolism,reduce the content of E-selectin and P-selectin,reduce inflammatory stimulation,and improve joint functional status,with significant clinical effects.
