摘 要:目的比较螺旋桨技术弥散加权成像(periodically rotated overlapping parallel lines with enhanced reconstruction diffusion-weighted imaging,Propeller-DWI)和单次激发平面回波弥散加权成像(single-shot echo-planar imaging diffusion-weighted imaging,SS-EPI-DWI)在装有金属假牙患者颅脑成像中的应用效果,评估Propeller-DWI在含有不可拆卸金属假牙患者中颅脑成像的优势和可行性。方法采用单盲自身对照方法,选择2021年3~7月在浙江省舟山市普陀区人民医院进行颅脑扫描的装有金属假牙且无磁共振禁忌证的37例患者为研究对象。对37例患者进行颅脑扫描,在常规轴位SS-EPI-DWI序列后加扫轴位Propeller-DWI序列。按照扫描序列不同,将37例患者的图像分为试验组(Propeller-DWI组)和对照组(SS-EPI-DWI组)。对两组图像的解剖结构显示、伪影及T2FSE和DWI融合图匹配率进行评分;计算两组图像中图像综合质量、玻璃体的几何变形率(GDR)、测量右侧基底节区的表观扩散系数(ADC)值,并进行秩和检验、配对t检验,以P<0.05表示差异具有统计学意义。结果试验组图像解剖结构的显示能力、伪影、融合图匹配率和图像综合质量的评分均优于对照组图像,其差异均具有统计学意义(均P<0.05)。试验组图像玻璃体的GDR显著低于对照组图像,其差异具有统计学意义(P<0.05);试验组图像右侧基底节区的ADC值[(972.03±36.14)mm^(2)/s]高于对照组图像[(873.86±34.75)mm^(2)/s],其差异具有统计学意义(P<0.05)。结论对于装有金属假牙的患者,其颅脑Propeller-DWI图像质量优于SS-EPI-DWI,可以取代SS-EPI-DWI进行应用。Objective To compare the application of periodically rotated overlapping parallel lines with enhanced reconstruction diffusion-weighted imaging(Propeller-DWI)and single-shot echo-planar imaging diffusion-weighted imaging(SS-EPI-DWI)in the brain images of the patients with metal dentures,so as to evaluate the advantages and feasibility of Propeller-DWI in brain images of the patients with non-removable metal dentures.Methods The single-blind self-control method is adopted.37 patients with metal dentures who had no contraindications to MRI were selected as the research objects.They received craniocerebral scans in Putuo District People’s Hospital of Zhoushan City,Zhejiang Province from March to July 2021.All the 37 patients received brain scans and axial Propeller-DWI sequences were further performed after conventional axial SS-EPI-DWI sequences.Based on different scanning sequences,the images of 37 patients were divided into two groups:Propeller-DWI group(experimental group)and SS-EPI-DWI group(control group).With regard to the images of both groups of patients,the anatomical structure display,artifact and the matching rate of T2 FSE and DWI fusion map were scored;the comprehensive image quality and the geometric deformation rate(GDR)of the vitreous body in both groups of images,as well as the apparent diffusion coefficient(ADC)value of the right basal ganglia region were calculated with the rank sum test and paired t test,the result of P<0.05 indicated that the difference was of statistical significance.Results The scores of the display ability,artifact,fusion map matching rate and comprehensive quality in the experimental group were better than those of the control group,and the difference was of statistical significance(all P<0.05).The vitreous body GDR of the images in the experimental group was significantly lower than that of the control group,and the difference was of statistical significance(P<0.05);the ADC value of the right basal ganglia region of the image in the experimental group[(972.03±36.14)mm^(
