“针向病所”针法治疗运动员膝关节肌腱止点周围病23例

摘　　要：目的:介绍“针向病所”理念下的一种膝关节肌腱止点周围病的针刺治疗新方法。方法:应用毫针斜刺电针受累肌肉法治疗国家队运动员膝关节肌腱止点周围病。采用疼痛视觉模拟量表(VAS)评分、激惹点查体变化、伤病运动员运动表现评价表、运动员膝关节肌腱止点周围病疗效评价标准评价疗效。结果:23例膝关节肌腱止点周围病运动员对应肌肉上均触及激惹点,经斜刺电针法治疗未出现针刺后膝关节止点区域疼痛加重及其他针刺不良反应。首次针刺2周后,对应肌肉激惹点压痛程度、触感异常程度、VAS评分均较前降低,运动表现评分较前升高,差异有统计学意义(P<0.001)。疗效评价显示:显效18例,有效5例,无效0例,总有效率为100%。结论:“针向病所”理念下的斜刺电针受累肌肉法是运动员膝关节肌腱止点周围病的有效治疗手段之一,疗效较好。Objective:To introduce a new acupuncture treatment method under the concept of “acupuncture onto the lesion” on the treatment of athletes’ tendinopathy around the knee.Methods:Based on the concept of “acupuncture onto the lesion”,the oblique electroacupuncture on the corresponding muscles method was used to treat the tendinopathy disease around the knee of athletes.The pain visual analogue scale(VAS) scores, physical examination changes at myofascial trigger points, sports performance evaluation form of injured athletes, and evaluation standard of therapeutic efficacy of athletes’ tendinopathy around the knee were used to evaluate the curative efficacy.Results:The myofascial trigger points and tenderness were palpated on the corresponding muscles of all the 23 athletes.There was no pain aggravation and other adverse acupuncture reactions in the knee joint insertion area after acupuncture.2 weeks after the first acupuncture, the level of tenderness and abnormal sense of tension on myofascial trigger points of the corresponding muscle were lower than that before treatment, and the difference was statistically significant(P<0.001).The VAS score was lower than that before treatment, and the difference was statistically significant(P<0.001).The sports performance score was higher than that before, and the difference was statistically significant(P<0.05).Efficacy evaluation showed that: 18 cases were markedly effective, 5 cases were effective, 0 was ineffective, and the total effective rate was 100%.Conclusion:The oblique electroacupuncture on the corresponding muscles based on the concept of “acupuncture onto the lesion” is one of the treatment methods for athletes’ tendinopathy around the keen, and it can achieve good results.
