超声引导下小针刀治疗髂胫束综合征60例

摘　　要：目的:观察超声引导下小针刀治疗髂胫束综合征(ITBS)的临床疗效。方法:回顾性分析2020年12月至2022年2月就诊的60例髂胫束综合征患者,采用超声引导下小针刀治疗,7 d为1个疗程,共治疗3个疗程。记录并分析治疗前、首次疗程后及疗程结束后3周的疼痛视觉模拟量表(VAS)评分、下肢功能指数评分(LEFS)及股骨外侧髁处髂胫束弹性模量。结果:患者疗程结束后3周VAS评分为(1.05±0.79)分,低于治疗前的(6.35±1.30)分及首个疗程后的(2.93±0.84)分;疗程结束后3周LEFS为(74.87±4.95)分,高于治疗前的(45.32±10.38)分与首个疗程后的(62.88±7.77)分;疗程结束后3周股骨外侧髁处髂胫束弹性模量为(9.41±5.94)Pa,低于治疗前的(14.21±7.85)Pa与首个疗程后的(11.64±6.33)Pa,差异均有统计学意义(P<0.05)。临床治疗总有效率为93.33%。结论:对于髂胫束综合征的患者,采用超声引导下小针刀疗法可以取得较好的临床疗效,能缓解患者疼痛,明显改善膝关节功能障碍,具有治疗精准、起效快、疗效好等优势,值得临床进一步推广应用。Objective:To observe the clinical efficacy of acupotomy under ultrasound guidance on the treatment of iliotibial band syndrome(ITBS).Methods:60 patients with iliotibial band syndrome treated from December 2020 to February 2022 were analyzed retrospectively.They were treated with acupotomy under ultrasound guidance with 7 d as a course of treatment and three courses in total.The visual analogue scale(VAS) score, lower extremity function index score(LEFS) and iliotibial tract elastic modulus at the lateral femoral condyle were recorded and analyzed before treatment, after the first treatment and 3 weeks after treatment.Results:The VAS score(1.05±0.79) points at 3 weeks after treatment was lower than that before treatment(6.35±1.30) points and after the initial treatment(2.93±0.84) points.The LEFS at 3 weeks after treatment(74.87±4.95) points was higher than before treatment(45.32±10.38) points and after initial treatment(62.88±7.77) points.3 weeks after the end of treatment, the elastic modulus of the iliotibial band at the lateral femoral condyle(9.41±5.94) Pa was lower than that before treatment(11.64±6.33) Pa and after the initial treatment(14.21±7.85) Pa.The difference was statistically significant(P<0.05).The total effective rate of clinical treatment was 93.33%.Conclusion:Acupotomy therapy under ultrasound guidance on the treatment of iliotibial band syndrome has achieved good clinical efficacy.It can effectively alleviate patients’ pain, significantly improve knee dysfunction, and has the advantages of accurate treatment, fast onset and good curative efficacy.It is worthy of further clinical application.
