摘 要:目的了解深圳市某社区居民流感疫苗接种对减少家庭内流感样病例传播的作用,评价不同人群接种流感疫苗的效果,尝试调查疫苗重点推广人群,为控制流感家庭内传播,降低家庭内流感样病例发生率提供科学依据.方法采用回顾性队列研究的调查方法选择深圳市某社区符合条件的居民进行电话调查,收集接种流感疫苗居民的基本信息及发病人数.结果流感样病例家庭内传播率为6.13%(19/310),其中接种组共5户家庭发生了家庭内传播,传播率为3.23%(5/155);对照组共14户家庭发生了家庭内传播,传播率为9.03%(14/155).接种组的家庭内传播率明显低于对照组的家庭内传播率(χ2=4.542,P<0.05).疫苗家庭内保护率为64.23%.不同年龄段接种组发生家庭内传播的二代病例发病之间有统计学差异(χ2=10.749,P<0.05).结论流感疫苗对预防家庭内流感样病例传播有一定的保护作用,18-60岁组接种可能二代发病最少.Objective To understand the role of influenza vaccination among communiyt residents in a district of Shenzhen in reducin g the spread of influenza in families,to evaluate the effectivesnse of influenza vaccination among different groups of people,da ntry to survey the key population for vaccination promotion,to control the separd of influenza in the family to reduce the incidence of agggarteed influenza in the family to provide scientific basisM.ethods A retrospective cohort study was conducted among eligible redseints in a community of Shenzhen to collect basic information and incidence of influenaz vaccination.Results The transmission rate of influenza-like illness in families was 6.13%(19/310),of which 5 families in vaccinatiognr oup had family transmission,and the transmission rate was 33.%2(5/155);in the control group,14 families had intra family transmission,an dthe transmission rate was 9.03%(14/155).The intra family trsamnission rate of the vaccination group was significantly lower than th aotf the control group.The protection rate of vaccine in familwya s 64.23%.There were significant differences in the incidence of second-egneration cases in different age groupsC.onclusion Influenza vaccine has a certain protective effect on preventing the spread of ilnufenza-like illness in families,and the second generation may avhe the least incidence in the 18-60 years old group.
