摘 要:目的探讨不同时间开始使用清肺排毒颗粒治疗新型冠状病毒肺炎(简称“新冠肺炎”)奥密克戎变异毒株无症状感染者的疗效。方法回顾性收集2022年3月8—24日在山东省青岛市红岛方舱医院收治的服用清肺排毒颗粒的新冠肺炎奥密克戎变异毒株无症状感染者病例资料。根据核酸检测阳性后开始服药时间分为≤3天组、4~6天组、7~9天组、≥10天组,比较各组患者基线资料、核酸转阴时间,采用多元线性回归分析年龄、开始服药不同时间对核酸转阴时间的影响,并记录不良事件发生及转归情况。结果共纳入患者111例,其中男性50例,女性61例,年龄中位数35岁,有合并症者12例。≤3天组4例,4~6天组57例,7~9天组42例,≥10天组8例,各组患者年龄、性别及合并症方面比较差异无统计学意义(P>0.05)。核酸转阴时间:≤3天组为3.0(2.3,3.0)天,4~6天组为7.0(7.0,8.0)天,7~9天组为9.0(8.0,10.0)天,≥10天组为11.0(11.0,12.0)天,各组核酸转阴时间比较差异有统计学意义(P<0.001)。多元线性回归结果显示,年龄与核酸转阴时间无相关性(β=0.008,P=0.886);开始服药时间与核酸转阴时间呈正相关,每延迟1天用药,核酸转阴天数增加0.885天(P<0.001,调整R^(2)=0.8)。所有患者在服药期间未出现严重不良事件,无患者转为确诊病例,均痊愈出院。结论新冠肺炎奥密克戎变异毒株无症状感染者越早开始服用清肺排毒颗粒治疗,核酸转阴时间越短。Objective To explore the association between Qingfei Paidu Granules(清肺排毒颗粒,QFPDG)initiated at different time points and clinical outcomes in asymptomatic patients infected with Omicron variant of coronavirus disease 2019(COVID-19).Methods This retrospective study recruited asymptomatic Omicron COVID-19 cases who have been treated by QFPDG from Qingdao Hongdao fangcang shelter hospital from 14 th March to 19 th March,2022.Patients were divided into four groups:≤3 days group,4-6 days group,7-9 days group and≥10 days group.The baseline data and duration of viral shedding were compared between groups,multivariate linear regression was used to analyze the influence of age and treatment initiation time on duration of viral shedding.The adverse events and prognosis of the patients were recorded.Results A total of 111 asymptomatic patients were included.Of them,50 were males and 61 were females,with the median age of 35 years.Twelve patients had comorbidities.There were four cases in the≤3 days group,57 cases in the 4-6 days group,42 cases in the 7-9 days group,and 8 cases in the≥10 days group,having no significant differences in age,gender and comorbidities at baseline(P>0.05).The median duration of viral shedding was 3.0(2.3,3.0)days,7.0(7.0,8.0)days,9.0(8.0,10.0)days,and 11.0(11.0,12.0)days in the groups of≤3 days,4-6 days,7-9 days and≥10 days,respectively,with significant difference between groups(P<0.001).The multivariate linear regression showed that there was no correlation between age and duration of viral shedding(β=0.008,P=0.886),while the medication initiation time was positively correlated with duration of viral shedding(P<0.001,adjusted R^(2)=0.8),suggesting that duration of viral shedding increased by 0.885 days as medication was initiated one day later.No serious adverse events occurred during medication,and none of the patients were converted into confirmed cases and were cured and discharged from hospitals.Conclusion Early treatment with QFPDG is linked to a shorter duration of v
