摘 要:目的:以锥形束计算机断层扫描(cone beam CT,CBCT)六维度匹配结果为参考,探讨胸腰椎在人体定位袋固定下的摆位误差对比分析。方法:选取2019年1月—2022年2月在复旦大学附属中山医院采用调强放疗技术治疗的胸腰椎肿瘤患者共29例,统计分析不同分段类型患者的六维度摆位误差及方差,分析不同分段类型纠正摆位误差前后CBCT匹配结果及其偏离度。结果:脊柱骨转移在患者身上的标记线保护完好、技术员对线准确的情况下,对平移的三个方向上(x、y、z)和旋转Pitch(Rx)、Roll(Ry)、Yaw(Rz)三个方向上的摆位误差中,其结果如下。胸椎:x(-0.39±2.61)mm、y(0.26±3.25)mm、z(-0.09±1.30)mm、Rx(-0.10±1.07)°、Ry(-0.22±0.99)°、Rz(-0.16±0.81)°;腰椎:x(-0.10±2.81)mm、y(1.18±2.50)mm、z(0.14±1.48)mm、Rx(0.23±0.49)°、Ry(0.23±0.66)°、Rz(-0.40±0.88)°。在胸椎、腰椎骨转移Rx、Ry的方向上摆位误差的对比差异较为显著(P<0.05),而其中胸椎的Rx、Ry误差比腰椎大。结论:人体定位袋对于胸腰椎骨的固定较为良好,重复性高。但对胸椎的固定作用比腰椎差。Objective To compare and analyze the positioning errors of thoracic and lumbar with positioning bag fixation based on cone beam CT(CBCT)six-dimensional matching results.Methods From January 2019 to February 2022,29 patients with thoracolumbar tumors treated with intensity modulated radiation therapy in Zhongshan Hospital Affiliated to Fudan University were selected.The six-dimensional positioning errors and variances of patients with different segmentation types were statistically analyzed,and the CBCT matching results and their deviations before and after correcting the positioning errors of different segmentation types were analyzed.Results Under the condition that the marking lines of spinal bone metastasis on patients were well protected and the technicians were accurate,the positioning errors in the three directions of translation(x,y,z)and rotation Pitch(Rx),Roll(Ry)and Yaw(Rz),the results were as follows.Thoracic vertebra:x(-0.39±2.61)mm,y(0.26±3.25)mm,z(-0.09±1.30)mm,Rx(-0.10±1.07)°,Ry(-0.22±0.99)°,Rz(-0.16±0.81)°;Lumbar spine:x(-0.10±2.81)mm,y(1.18±2.50)mm,z(0.14±1.48)mm,Rx(0.23±0.49)°,Ry(0.23±0.66)°,Rz(-0.40±0.88)°.In the direction of Rx and Ry of thoracic and lumbar bone metastases,there was a significant difference in positioning errors(P<0.05),and the Rx and Ry errors of thoracic vertebrae were larger than those of lumbar vertebrae.Conclusion Positioning bag is good for fixation of thoracic and lumbar vertebrae with high repeatability.But the fixation effect on thoracic vertebra is worse than that on lumbar vertebra.
