摘 要:目的探讨对老年慢性阻塞性肺疾病(COPD)患者实施基于心肺运动试验的运动处方训练的效果。方法选取2019年11月至2021年11月南京医科大学附属淮安第一医院收治128例老年COPD患者,采用随机数字表法将其分为对照组(64例)、试验组(64例)。对照组实施常规护理,试验组在对照组基础上实施基于心肺运动试验的运动处方训练,两组均干预6周。比较两组干预前后运动耐力、肺功能及生活质量的变化。结果干预后,两组6 min步行测试、运动持续时间、最大运动功率、用力肺活量(FVC)、第1秒用力呼气容积(FEV_(1))、FEV_(1)/FVC均高于干预前,且试验组高于对照组(P<0.05)。干预后,两组圣乔治呼吸问卷的症状、活动受限、疾病评分均低于干预前,且试验组低于对照组(P<0.05)。结论在老年COPD患者中应用基于心肺运动试验的运动处方训练可提高其运动耐力,促进肺功能的恢复,改善生活质量。Objective To investigate the effect of exercise prescription training based on cardiopulmonary exercise test in elderly patients with chronic obstructive pulmonary disease(COPD).Methods A total of 128 elderly patients with COPD admitted to Huaian First Hospital Affiliated to Nanjing Medical University from November 2019 to November 2021 were selected and divided into control group(64 cases)and experimental group(64 cases)by random number table method.The control group was given routine nursing,while the experimental group was given exercise prescription training based on cardiopulmonary exercise test on the baseic of control group.Both groups were intervened for six weeks.The changes of exercise endurance,lung function,and quality of life before and after intervention were compared between the two groups.Results After intervention,the six-minute walking test,exercise duration,maximum exercise power,forced vital capacity(FVC),forced expiratory volume in one second(FEV_(1)),and FEV_(1)/FVC in the two groups were higher than those before intervention,and the experimental group was higher than that in the control group(P<0.05).After intervention,the symptoms,activity limitation,and disease scores of St.George’s respiratory questionnaire in the two groups were lower than those before intervention,and the experimental group was lower than the control group(P<0.05).Conclusion In elderly patients with COPD,exercise prescription training based on cardiopulmonary exercise test can improve exercise endurance,promote the recovery of lung function and improve the quality of life.
