摘 要:糖尿病足(DF)是长期高血糖导致的外周神经病变、血管病变因素等相互作用的疾病。也是导致患者感染甚至截肢的重要原因。抑制相关炎症通路和促进坏死组织脱落和新生组织愈合是治疗的关键,DF发生发展主要涉及高血糖导致的血管生成受损、炎症反应异常、神经病变等。其中细胞因子信号转导抑制物(SOCS)可以调控蛋白酪氨酸激酶信号转导和转录活化因子(JAK-STAT)通路抑制相关炎症通路和促进坏死组织脱落和新生组织愈合,可以通过促进炎症因子的表达、诱导炎症细胞活化、在DF的发生发展中起着重要作用。大量文献研究显示,中药在防治DF中具有明显的优势。有些中药、中药提取物和中药复方能够调节SOCS信号通路或JAK-STAT通路,从而改善DF的炎症性损伤以及神经损伤,进而发挥保护作用。本研究针对近年来关于SOCS/JAK-STAT通路在DF中的研究进展及中药的干预作用现状做一综述,旨在为DF治疗和药物开发研究提供新的思路。Diabetic foot(DF)is a disease of interaction between peripheral neuropathy and vascular disease caused by long-term hyperglycemia.It is also an important cause of infections and even amputations.Inhibition of inflammatory pathways and promotion of shedding of necrotic tissue and healing of new tissue are the keys to treatment.The occurrence and development of DF mainly involves impaired angiogenesis,abnormal inflammatory response,neuropathy.Suppressor of cytokine signal(SOCS)can regulate the janus kinase/signal transducer and activators of transcription(JAK-STAT)pathway to inhibit related inflammatory pathways and promote the shedding of necrotic tissue and the healing of new tissue.It plays an important role in the occurrence and development of DF.A large number of literature studies show that traditional Chinese medicine has obvious advantages in DF prevention and treatment.Some Chinese medicines,Chinese herbal extracts and Chinese herbal compounds can regulate the SOCS signaling pathway,thereby improving the inflammatory damage of DF,and thus playing a protective role.In this paper,the research progress of SOCS pathway in DF and the intervention status of traditional Chinese medicine were reviewed in recent years,in order to provide new ideas for the treatment of DF and drug development research.
