摘 要:目的分析上海电力医院门诊药房处方退药的原因,保证用药安全,减少门诊退药。方法采用回顾性分析方法,选取2019年1月至6月上海电力医院门诊药房退药处方共145张作为研究对象,分析退药事件的发生原因、药物退回问题涉及的药物类型及科室,以期降低门诊药房退药率。结果2019年1月至6月门诊药房共调配处方66650张,其中退药处方145张,约占半年处方量的0.22%。药物退回原因主要为药物因素、医护因素以及患者因素,其中患者因素占总退回因素的48.96%。门诊药房发生的药物退回问题涉及药物类型、退药科室较多,其中心脑血管药退回占比最高,为40.00%,退药科室内科占比最高,为29.66%。结论导致门诊药房退药现象原因较为繁杂多样,药房人员应完善门诊退药操作流程,规范并严格执行退药制度,加强医患沟通,提高患者对医生的信任感与对药物的依从性,药房人员也应和医师之间及时沟通,促进合理用药,减少可控因素的退药,保障用药安全,从而降低退药现象的发生率。Objective To analyze the reasons of prescription withdrawal in Outpatient Pharmacy of Shanghai Electric Power Hospital,so as to ensure drug safety and reduce drug withdrawal in Outpatient Department.Methods A total of 145 prescriptions returned from outpatient pharmacy of Shanghai Electric Power Hospital from January to June 2019 were selected as the research objects by retrospective analysis.The causes of drug return events,drug types and departments involved in drug return problems were analyzed,in order to reduce the rate of drug return in outpatient pharmacy.Results From January to June 2019,a total of 66650 prescriptions were dispensed in outpatient pharmacy,among which 145 prescriptions were returned,accounting for 0.22%of the total prescriptions in half a year.The reasons for drug return were mainly drug factors,medical factors and patient factors,among which patient factors accounted for 48.96%of the total return factors.The drug return problems occurred in Outpatient Pharmacy were more related to drug types and drug return departments,with the highest proportion of cerebrovascular drug return in the center(40.00%)and the highest proportion of internal medicine in the drug return department(29.66%).Conclusion The causes of Outpatient Pharmacy drug return are complex and diverse.Pharmacy personnel should improve the outpatient drug return operation process,standardize and strictly implement the drug return system,strengthen communication between doctors and patients,improve patients'trust in doctors and compliance with drugs.Pharmacy personnel should also communicate with doctors in a timely manner to promote rational drug use,reduce drug returns due to controllable factors,ensure drug safety,and reduce the incidence of drug return.
