我国医保儿童专用药发展历程与前景分析

摘　　要：目的:改善儿童用药保障,为医保药品目录中儿童专用药的调整提供建议。方法:通过医保目录检索和文献检索相结合的方式,对比2017年—2021年四版医保目录中儿童专用药的调整过程,了解儿童专用药保障的进展和存在的问题,为进一步完善我国医保目录中儿童专用药的调整提供建议。结果:儿童专用药种类增加了51.52%,经过国家谈判的儿童专用药从0增加至11种,但在2021版医保目录中,儿童专用药占西药比重仅为3.36%,总量依然不足。儿童专用药适宜剂型发展缓慢,用法用量描述不够严谨准确,未充分考虑儿童用药实际需求,药品适宜性不足。结论:建议采取激励措施促进药企对儿童专用药的研发和生产,丰富儿童专用药的剂型、规格和口味,增加儿童专用药的数量,加大对儿童专用药适宜性的调整权重,改善我国儿童用药的供应保障。Objective:The paper is to improve drug security for children,and to provide suggestions for the adjustment of special drugs for children in the national catalog of drugs covered by basic medical insurance.Methods:Through the combination of medical insurance catalog retrieval and literature retrieval,the adjustment process of special drugs for children in the four editions of medical insurance catalog from 2017 to 2021 was compared to understand the progress and problems of special drugs for children,and provide suggestions for further improving the adjustment of special drugs for children in the national medical insurance catalog.Results:The types of special drugs for children increased by 51.52%,and the number of special drugs with national negotiation for children increased from 0 to 11.However,in the 2021 version of the medical insurance catalogue,the proportion of special drugs for children in western drugs was only 3.36%,which was still insufficient.The development of appropriate dosage forms for special drug use in children is slow,the description of usage and dosage is not precise and accurate enough,the actual needs of children are not fully considered,and the suitability of drugs is insufficient.Conclusions:It is suggested to take incentive measures to promote the research,development and production of special drugs for children,enrich the dosage forms,specifications and tastes of special drugs for children,increase the number of special drugs for children,increase the adjustment weight of the suitability of special drugs for children,and improve the supply guarantee of children's drugs.
