基于红外热成像技术对任脉灸联合手法治疗寒凝血瘀型原发性痛经的疗效观察

摘　　要：目的:基于红外热成像技术观察任脉灸联合手法治疗寒凝血瘀型原发性痛经(PD)的临床疗效。方法:随机将72例寒凝血瘀型PD患者分为治疗组与对照组,每组36例。对照组给予布洛芬缓释胶囊治疗,治疗组给予任脉灸联合手法治疗。采用视觉模拟评分法(VAS)评分记录2组治疗前后腹痛程度,采用红外热像仪记录2组治疗前后任脉平均温度,检测2组子宫动脉阻力指数(RI)、搏动指数(PI)、收缩期峰值/舒张期峰值(A/B),比较2组临床疗效和不良反应发生率。结果:治疗后,2组VAS评分均较治疗前降低(P<0.05),任脉平均温度均较治疗前升高(P<0.05);治疗组VAS评分低于对照组(P<0.05),任脉平均温度高于对照组(P<0.05)。治疗后,2组子宫动脉PI、RI、A/B均较治疗前降低(P<0.05),治疗组子宫动脉PI、RI、A/B均低于对照组(P<0.05)。治疗组总有效率97.22%,高于对照组80.56%(P<0.05)。治疗组不良反应发生率8.33%,低于对照组27.78%(P<0.05)。结论:任脉灸联合手法治疗寒凝血瘀型PD疗效确切,能促进任脉气血运行,缓解疼痛,改善子宫动脉血流,减少不良反应。Objective: To discuss the clinical effect of conception vessel moxibustion combined with manipulation for primary dysmenorrhea(PD) of cold congealing and blood stasis type based on infrared thermal imaging technology. Methods: A total of 72 cases of PD patients of cold congealing and blood stasis type were randomly divided into the treatment group and the control group,with 36 cases in each group. The control group was treated with ibuprofen sustained-release capsules,and the treatment group was treated with conception vessel moxibustion combined with manipulation. Before and after treatment,Visual Analogue Scale(VAS) scores were used to record the abdominal pain in the two groups, and the infrared thermal imager was applied to record the average temperature of conception vessel. The values of resistance index(RI), pulsatility index(PI) and systolic peak/diastolic peak(A/B) in uterine artery in the two groups were detected. The clinical effects and the incidence of adverse reactions in the two groups were compared. Results:After treatment,the VAS scores in the two groups were decreased when compared with those before treatment(P<0.05),and the average temperature of conception vessel was increased(P<0.05). The VAS score in the treatment group was lower than that in the control group(P<0.05),and the average temperature of conception vessel was higher(P<0.05). After treatment,the values of RI,PI and A/B in uterine artery in the two groups were decreased when compared with those before treatment(P<0.05),and the values of RI,PI and A/B in uterine artery in the treatment group were lower than those in the control group(P<0.05). The total effective rate was 97.22% in the treatment group, higher than that of80.56% in the control group(P<0.05). The incidence of adverse reactions was 8.33% in the treatment group, lower than that of 27.78% in the control group(P<0.05).
