摘　　要：目的研究水通道蛋白-5(AQP5)基因多态性与脓毒症易感性及预后的关系。方法选取2017年11月-2020年11月于医院收治的100例脓毒症患者作为脓毒症组,另外选取80例同期医院普通感染患者作为非脓毒症组,提取两组患者DNA,经聚合酶链式扩增反应(PCR)及测序技术检测AQP5基因多态性,分析各位点多态性与脓毒症易感性、预后的关系,采用Logistic分析脓毒症患者预后的影响因素。结果脓毒症组的急性生理和慢性健康状况评分系统Ⅱ(APACHEⅡ)评分、PCT水平高于非脓毒症组(P<0.05);脓毒症组和非脓毒症组rs3736309、rs296759等位基因和基因型分布频率比较,差异均无统计学意义;AQP5rs3736309、rs296759位点基因多态性与脓毒症感染无相关性;存活组与死亡组年龄、是否脓毒症休克、APACHEⅡ评分、病情程度、rs296759位点基因型比较,差异有统计学意义(P<0.05);脓毒症休克(OR=1.650)、APACHEⅡ评分≥15分(OR=1.533)、rs296759基因型(CC)(OR=1.608)是影响脓毒症患者预后的独立危险因素(P<0.05)。结论AQP5基因2个位点rs3736309、rs296759的多态性与脓毒症易感性无明显相关,但rs296759位点多态性与脓毒症预后相关,CC基因型可能提示脓毒症预后恶化。OBJECTIVE To study the correlations of aquaporin-5(AQP5)gene polymorphisms with sepsis susceptibility and prognosis.METHODS A total of 100 patients with sepsis admitted to hospital between Nov 2017 and Nov 2020 were enrolled as the sepsis group,and the other 80 patients with common infection during the same period were enrolled as the non-sepsis group.DNAs from both groups were extracted as the template to detect AQP5 gene polymorphisms by polymerase chain reaction(PCR)assay and sequencing technology.The correlations of AQP5 gene polymorphisms at each locus with the sepsis susceptibility and prognosis were analyzed.The influencing factors affecting prognosis in the sepsis patients were analyzed by Logistics analysis.RESULTS The scores of Acute Physiology and Chronic Health EvaluationⅡ(APACHEⅡ)and procalcitonin(PCT)level in the sepsis group were significantly higher than those in the non-sepsis group(P<0.05).There was no significant difference in the distribution frequencies of alleles and genotypes at rs3736309 and rs296759 loci between the two groups.The polymorphisms of AQP5 gene at rs3736309 and rs296759 loci were not correlated with sepsis infection.There were significant differences in age,septic shock,APACHEⅡscore,disease severity,and rs296759 genotype between the survival group and the death group(P<0.05).Septic shock(OR=1.650),APACHEⅡscore≥15 points(OR=1.533)and CC genotype at rs296759 locus(OR=1.608)were independent risk factors affecting prognosis of sepsis patients(P<0.05).CONCLUSION The polymorphisms of AQP5 gene at two loci are not significantly correlated with sepsis susceptibility.However,polymorphisms at rs296759 locus are related to sepsis prognosis.CC genotype may indicate poor prognosis of sepsis.
