摘  要:目的分析T细胞免疫球蛋白黏蛋白分子3(TIM-3)、白细胞介素-27(IL-27)、甘露糖结合凝集素相关丝氨酸蛋白酶1(MASP1)在肝包虫病合并乙型肝炎病毒(HBV)感染患者中的表达情况及临床意义。方法选取2020年3月-2021年3月内蒙古民族大学附属医院蒙医外科收治的肝包虫病患者168例,根据是否合并HBV感染分为非感染组89例、感染组79例。另外同期选取于医院行健康体检的健康志愿者80名作为健康组。检测血清TIM-3、IL-27水平及外周血单个核细胞中MASP1表达量,对比分析在三组研究对象中的表达差异及在不同包虫类型、肝功能分级中的表达差异。结果感染组TIM-3、IL-27水平、MASP1表达量高于非感染组、健康组(P<0.05)。TIM-3、IL-27水平、MASP1表达在感染组不同包虫类型中比较,差异无统计学意义。肝功能分级为B~C级的患者TIM-3、IL-27水平、MASP1表达量高于A级患者(P<0.05)。受试者工作特征曲线(ROC)分析显示,与TIM-3、IL-27、MASP1单项诊断相比,三项联合对肝包虫病合并HBV感染的诊断价值较高(P<0.05)。结论TIM-3、IL-27、MASP1在肝包虫病合并HBV感染中表达升高,且与患者肝功能损伤程度相关,临床可根据其指标表达高低评估疾病。OBJECTIVE To analyze the expression and clinical significance of T cell immunoglobulin mucin molecule 3(TIM-3),interleukin-27(IL-27),and mannose-binding lectin-related serine protease 1(MASP1)in patients with hepatic echinococcosis complicated with hepatitis B virus(HBV)infection.METHODS Total of 168 patients with hepatic echinococcosis treated in the department of Mongolian Medicine in Affiliated Hospital of Inner Mongolia University for Nationalities from Mar 2020 to Mar 2021 were recruited.According to whether HBV infection occurred,they were divided into 89 cases in the non-infection group and 79 cases in the infection group.In addition,80 healthy volunteers who underwent physical examination in the hospital during the same period were in the health group.The levels of serum Tim-3,IL-27 and MaSp1 in peripheral blood mononuclear cells were detected.The expression differences in the expression among the three groups,different echinococcal types and liver function grades were compared.RESULTS The levels of Tim-3,IL-27 and the expression of MaSp1 in the infected group were significantly higher than those in the non-infected group and healthy group(P<0.05).There was no significant difference in the levels of Tim-3,IL-27 and MaSp1 among different echinococcal types in the infection group.The levels of Tim-3,IL-27 and MaSp1 in patients with liver function grade B-C were significantly higher than those in patients with grade A(P<0.05).The receiver operating characteristic curve(ROC)curve analysis showed that compared with the single indexes of Tim-3,IL-27 and MaSp1,the combination of the three indexes had significantly higher diagnostic value for hepatic echinococcosis complicated with HBV infection(P<0.05).CONCLUSION The expression levels of TIM-3,IL-27 and MASP1 increased in patients with hepatic echinococcosis complicated with HBV infection,which were related to the degree of liver function injury.The disease can be evaluated by the expression of TIM-3,IL-27 and MASP1.
