妇用抑菌片治疗人乳头瘤病毒持续感染患者的临床效果

摘　　要：目的探讨妇用抑菌片联合重组人干扰素对人乳头瘤病毒(HPV)持续感染患者的临床效果及对HPV清除率和阴道微环境的影响。方法前瞻性收集2020年1月-2020年7月上饶市立医院妇科诊治的HPV持续感染患者80例,随机数字表法分为对照组(n=41)、研究组(n=39),对照组采用重组人干扰素治疗,研究组给予妮可Ⅰ度妇用抑菌片联合重组人干扰素治疗,两组均持续治疗1个月。治疗结束后比较两组疗效和HPV清除率,观察治疗前后阴道微环境细胞因子水平变化及血清免疫功能指标变化,比较两组治疗后阴道微环境状况和药物安全性。结果治疗后研究组总有效率高于对照组,治疗后6个月HPV清除率较对照组高(P<0.05)。与治疗前比较,治疗后两组IL-17、IL-23、IL-10降低,且研究组低于对照组,两组IFN-γ升高,研究组高于对照组(P<0.05)。研究组阴道pH值≤4.5、乳酸杆菌为中或大量、Nugnet评分≤3分的占比高于对照组(P<0.05)。结论妮可Ⅰ度妇用抑菌片联合重组人干扰素在HPV持续感染患者治疗中有明确疗效,两者联合应用在有效提高患者HPV清除率的同时改善患者阴道微环境,并且有一定安全性。OBJECTIVE To investigate the clinical effects of antibacterial tablets combined with recombinant human interferon in the treatment of patients with persistent human papilloma virus(HPV)infection,and the influence on HPV clearance rate and vaginal microenvironment in women.METHODS The study prospectively included 80 patients with persistent HPV infection admitted to gynecology department of Shangrao Municipal Hospital between Jan and Jul 2020.The subjects were randomly assigned to receive one month of treatment with recombinant human interferon(as the control group,41 cases)or NicoleⅠ-degree woman antibacterial tablets combined with recombinant human interferon(as the observation group,39 cases).After treatment,curative effect and HPV clearance rate between the groups were compared.Changes of cytokine levels in the vaginal microenvironment and the changes of serum immune function indexes before and after treatment were observed.Vaginal microenvironment status and drug safety between the two groups after treatment were compared.RESULTS After treatment,the total response rate in the observation group was significantly higher than that in the control group,and the observation group had significantly higher HPV clearance rate than the control group at 6 months after treatment(P<0.05).After treatment,IL-17,IL-23,and IL-10 levels in the two groups decreased,and those in the study group was significantly lower than in the control group;IFN-γlevel increased in both groups and that in the study group was significantly higher than in the control group(P<0.05).The proportions of vaginal pH≤4.5,medium or large amount of Lactobacilli,and the Nugnet score≤3 were significantly higher in the observation group than those in the control group(P<0.05).CONCLUSION NicoleⅠ-degree antibacterial tablets for woman combined with recombinant human interferon are effective in the treatment of patients with persistent HPV infection.The combined treatment can effectively improve the HPV clearance rate and significantly improve the pat
