V-Y矫正术与“Z”瓣成形术在单睑伴内眦赘皮Park法治疗中的应用对比

摘　　要：目的:探讨V-Y矫正术与“Z”瓣成形术在单睑伴内眦赘皮Park法治疗中的应用效果。方法:回顾性分析2019年3月-2021年3月收治的62例单睑伴内眦赘皮就医者,根据手术方案分为V-Y矫正组30例和Z成形组32例。V-Y矫正组就医者实施V-Y矫正术联合Park法重睑成形术。Z成形组就医者实施“Z”瓣成形术联合Park法重睑成形术。比较两组的手术效果、术后瘢痕评分、满意度评分、并发症。结果:V-Y矫正组与Z成形组的治疗有效率分别为96.67%和93.75%,差异无统计学意义(P>0.05)。术后3个月、术后6个月、术后1年,两组就医者的瘢痕评分逐渐降低,并且V-Y矫正组的瘢痕评分低于Z成形组(P<0.05)。V-Y矫正组就医者满意度评分高于Z成形组(P<0.05)。V-Y矫正组与Z成形组的手术并发症率分别为6.67%、18.75%,差异无统计学意义(P>0.05)。结论:V-Y矫正术与“Z”瓣成形术联合Park法重睑成形术治疗单睑伴内眦赘皮均有良好的手术效果,不过V-Y矫正术的瘢痕更轻微、并发症更少。Objective This research aims to investigate the application effects of V-Y plasty and Z-plasty in the treatment of single eyelid with epicanthus by Park method.Methods This study retrospectively reviewed 62 patients with single eyelid and epicanthus treated between March 2019 and March 2021.According to the surgical plan,the patients were divided into V-Y plasty group(30 cases)and Z-plasty group(32 cases).Patients in the V-Y plasty group were treated with V-Y plasty combined with double eyelid plasty by Park method.Patients in the Z-plasty group were treated with Z-plasty combined with double eyelid plasty by Park method.The two groups were compared on surgical effect,postoperative scar score,satisfaction score,and complications.Results The treatment response rates in the V-Y plasty group and the Z-plasty group were 96.67% and 93.75%,respectively(P>0.05).No recurrence was observed in either the V-Y plasty group or the Z-plasty group during the 1-year follow-up.The scar scores of the two groups gradually decreased at 3 months,6 months,and 1 year after surgery.The V-Y plasty group had lower scar scores than the Z-plasty group(P<0.05).The patient satisfaction score in the V-Y plasty group was higher than that in the Z-plasty group(P<0.05).The incidence rates of surgical complication in the V-Y plasty group and the Z-plasty group were 6.67% and 18.75%,respectively(P>0.05).Conclusion Both of V-Y plasty and Z-plasty combined with double eyelid plasty by Park method can achieve good results in the treatment of single eyelid with epicanthus.However,the car caused by the former is milder and its complications are fewer.
