摘　　要：目的探讨关节镜下半月板成形缝合术治疗盘状半月板损伤的效果及对膝关节活动度的影响。方法选取我院收治的92例盘状半月板损伤患者,根据治疗方案不同将其分为全切组(46例,关节镜下全切术)和成形组(46例,关节镜下半月板成形缝合术)。比较两组的治疗效果。结果成形组的治疗优良率高于全切组,并发症总发生率低于全切组(P<0.05)。随访3个月后,两组的核转录因子-κB(NF-κB)、人类软骨糖蛋白-39(YKL-40)、基质金属蛋白酶-13(MMP-13)、基质金属蛋白酶-1(MMP-1)、骨钙素(BGP)水平均降低,且成形组低于全切组(P<0.05)。术后3、6个月,两组的膝关节活动度均增大,且成形组大于全切组(P<0.05)。结论关节镜下半月板成形缝合术治疗盘状半月板损伤的效果理想,能有效促进患者膝关节功能恢复。Objective To explore the effect of arthroscopic meniscus plasty and suture in the treatment of discoid meniscus injury and its influence on knee joint activity.Methods A total of 92 patients with discoid meniscus injury admitted in our hospital were selected,and the patients were divided into total resection group(46 cases,arthroscopic total resection)and forming group(46 cases,arthroscopic meniscus plasty and suture)according to different treatment schemes.The therapeutic effect of the two groups were compared.Results The excellent and good rate of treatment in the forming group was higher than that in the total resection group,and the total incidence of complications was lower than that in the total resection group(P<0.05).After 3 months of follow-up,the nuclear factor-κB(NF-κB),human cartilage glycoprotein-39(YKL-40),matrix metalloproteinase-13(MMP-13),matrix metalloproteinase-1(MMP-1)and bone gla protein(BGP)levels in the two groups decreased,and those in the forming group were lower than the total resection group(P<0.05).At 3 and 6 months after operation,the knee joint activity in the two groups increased,and that in the forming group was greater than the total resection group(P<0.05).Conclusion Arthroscopic meniscus plasty and suture in the treatment of discoid meniscus injury has an ideal effect,it can effectively promote the recovery of knee joint function.
