养正消积胶囊辅助放疗治疗晚期胃癌患者的近期疗效及作用机制

摘　　要：目的 观察养正消积胶囊辅助三维适形对晚期胃癌患者近期疗效的影响,并分析其作用机制。方法 选取2017年4月—2019年6月在河南省人民医院接受治疗的120例晚期胃癌患者进行回顾性分析,根据患者治疗方式的差异,将单纯接受三维适形放疗的患者纳入放疗组,在此基础上联用养正消积胶囊辅助治疗的患者纳入联合组,各60例。对两组患者的近期疗效、血清生化指标、不良反应等进行对比分析。结果 联合组客观缓解率(ORR)显著高于对照组(66.67%vs 33.33%,P=0.000);治疗后,联合组血清肿瘤标志物癌胚抗原(CEA)、癌抗原125(CA125)、糖链抗原199(CA199)水平均显著低于放疗组(P<0.05);血清肿瘤坏死因子(TNF-α)、白细胞介素-6(IL-6)、C反应蛋白(CRP)、血管内皮生长因子(VEGF)水平均显著低于放疗组(P<0.05);联合组胃肠道不良反应发生率显著低于放疗组(P<0.05);联合组患者生活质量显著高于放疗组(P<0.05)。结论 养正消积胶囊辅助三维适形可以提高晚期胃癌患者免疫功能,下调血清肿瘤标志物、炎性细胞因子与VEGF水平,提高晚期胃癌的近期疗效并延缓疾病进展,同时可以有效减轻患者胃肠道不良反应,提高患者的生活质量。Objective To observe the effect of Yangzheng Xiaoji capsule assisted 3 d conformal treatment on the short-term efficacy of advanced gastric cancer patients,and analyze the mechanism of action.Methods A total of 120 patients with advanced gastric cancer treated in our hospital from April 2017 to June 2019 were selected for retrospective analysis.According to the differences in the treatment methods of the patients,patients receiving only three-dimensional conformal radiotherapy were included in the radiotherapy group,and on this basis,patients receiving combined therapy with Yangzheng Xiaoji capsule were included in the combined group,60 patients in each group.The short-term efficacy,serum biochemical indexes and adverse reactions of the two groups were compared and analyzed.Results The objective remission rate(ORR) of the combined group was significantly higher than that of the control group(66.67% vs 33.33%,P=0.000).After treatment,the serum levels of tumor markers CEA,CA125 and CA199 in the combined group were significantly lower than those in the radiotherapy group(P<0.05).Serum levels of tumor necrosis factor(TNF-α),interleukin-6(IL-6),C-reactive protein(CRP) and vascular endothelial growth factor(VEGF) were significantly lower than those of the radiotherapy group(P<0.05).The incidence of gastrointestinal adverse reactions in the combined group was significantly lower than that in the radiotherapy group(P<0.05).The quality of life of the combined group was significantly higher than that of the radiotherapy group(P<0.05).Conclusion The three-dimensional conformation assisted by Yangzheng Xiaoji capsule can improve the immune function of patients with advanced gastric cancer,down-regulate the serum tumor markers,inflammatory cytokines and VEGF levels,improve the short-term efficacy and delay the disease progression of patients with advanced gastric cancer,and at the same time,effectively reduce the gastrointestinal adverse reactions of patients and improve the quality of life of patients.
