CysC、Hcy、U-mAlb联合RBP检测在早期糖尿病肾病诊断中的应用价值

摘　　要：目的:探讨胱抑素C(CysC)、同型半胱氨酸(Hcy)、尿微量白蛋白(U-mALB)联合视黄醇结合蛋白(RBP)检测在早期糖尿病肾病诊断中的应用价值。方法:回顾性分析2020年1月至2021年9月该院收治的232例糖尿病患者的临床资料,依据尿微量白蛋白/肌酐(U-mAlb/Cr)比值将其分为正常尿组(男性U-mAlb/Cr比值<22 mg/g,女性U-mAlb/Cr比值<31 mg/g)112例、糖尿病微量白蛋白尿组(22 mg/g≤男性U-mAlb/Cr比值<220 mg/g,31 mg/g≤女性U-mAlb/Cr比值<220 mg/g)76例、糖尿病大量白蛋白尿组(U-mAlb/Cr比值≥220 mg/g)44例,选取同期80名健康体检者作为对照组。比较对照组与糖尿病组血清CysC、Hcy、U-mAlb、RBP水平,不同糖尿病分期患者CysC、Hcy、U-mAlb、RBP水平,四组CysC、Hcy、U-mAlb、RBP单项检测及联合检测的阳性检出率。结果:糖尿病组血清CysC、Hcy、U-mAlb、RBP水平均高于对照组,差异有统计学意义(P<0.05);糖尿病大量白蛋白尿组血清CysC、Hcy、U-mAlb、RBP水平高于糖尿病微量白蛋白尿组和正常尿组,且糖尿病微量白蛋白尿组高于正常尿组,差异有统计学意义(P<0.05);血清CysC、Hcy、U-mAlb、RBP单项检测及联合检测的阳性检出率:糖尿病大量白蛋白尿组>糖尿病微量白蛋白尿组>正常尿组>对照组,差异有统计学意义(P<0.05)。结论:早期糖尿病肾病患者血清CysC、Hcy、U-mAlb、RBP水平均升高,CysC、Hcy、U-mAlb、RBP联合检测对早期糖尿病肾病具有较高的诊断价值。Objective:To observe the application effects of detection of cystatin C(CysC),homocysteine(Hcy),urinary microalbumin(U-mALB)combined with retinol binding protein(RBP)in the diagnosis of early diabetic nephropathy.Methods:The clinical data of 232 diabetic patients admitted to the hospital from January 2020 to September 2021 were retrospectively analyzed.According to the urinary microalbumin/creatinine(U-mAlb/Cr)ratio,they were divided into normal urine group(male U-mAlb/Cr ratio<22 mg/g,female U-mAlb/Cr ratio<31 mg/g,112 cases),diabetic microalbuminuria group(22 mg/g≤male U-mAlb/Cr ratio<220 mg/g,31 mg/g≤female U-mAlb/Cr ratio<220 mg/g,76 cases),diabetic macroalbuminuria group(U-mAlb/Cr ratio≥220 mg/g,cases).80 healthy subjects were selected as control group.The levels of serum CysC,Hcy,U-mAlb and RBP were compared between the control group and the diabetic group.The levels of CysC,Hcy,U-mAlb and RBP in the patients with different stages of diabetes were compared.The positive detection rates of single CysC,single Hcy,single U-mAlb,single RBP and combine detection in the four groups were compared.Results:The levels of serum CysC,Hcy,U-mAlb and RBP in the diabetic group were higher than those in the control group,and the differences were statistically significant(P<0.05).The levels of serum CysC,Hcy,U-mAlb and RBP in the diabetic macroalbuminuria group were higher than those in the diabetic microalbuminuria group and the normal urine group,those in the diabetic microalbuminuria group were higher than those in the normal urine group,and the differences were statistically significant(P<0.05).The positive detection rate of single serum CysC,single Hcy,single U-mAlb,single and the combined detection were diabetic macroalbuminuria group>diabetic microalbuminuria group>normal urine group>control group,and the differences were statistically significant(P<0.05).Conclusions:The levels of serum CysC,Hcy,U-mAlb and RBP in the patients with early diabetic nephropathy are increased.The combined detection of CysC,Hcy,U-mAlb
