摘 要:目的:探讨Teach-back健康教育联合延续性护理对心脏瓣膜置换患者术后康复的影响。方法:选取2019年6月至2021年6月于该院行瓣膜置换术治疗的94例心脏瓣膜病患者进行前瞻性研究,按照随机数字表法分为观察组和对照组,各47例。对照组实施延续性护理,观察组实施Teach-back健康教育联合延续性护理,两组均护理3个月。比较两组护理前后心功能[6 min步行距离(6MWD)、步频]、自我护理能力[自我护理能力量表(ESCA)]、负性情绪[汉密尔顿焦虑量表-14(HAMA-14)、汉密尔顿抑郁量表-17(HAMD-17)]、生命质量[生活质量综合评定问卷(GQOLI-74)]及护理满意度。结果:观察组护理后6MWD、步频、ESCA评分、GQOLI-74各维度评分及总分均高于对照组,差异有统计学意义(P<0.05);观察组护理后HAMA-14、HAMD-17评分均低于对照组,差异有统计学意义(P<0.05);观察组护理满意度各项评分及总分均高于对照组,差异有统计学意义(P<0.05)。结论:Teach-back健康教育联合延续性护理应用于心脏瓣膜置换患者可改善术后心功能,提升自我护理能力,缓解负性情绪,提高生命质量及护理满意度。Objective:To explore effects of Teach-back health education combined with continuous nursing on postoperative rehabilitation of patients undergoing cardiac valve replacement surgeries.Methods:A prospective study was conducted on 94 patients with valvular heart disease who underwent valve replacement surgeries in the hospital from June 2019 to June 2021.They were divided into observation group and control group according to the random number table method,47 cases in each.The control group was given continuous nursing,while the observation group was given the Teach-back health education combined with continuous nursing.Both groups were treated for 3 months.The cardiac function[6 min walking distance(6 MWD),step frequency],the self-care ability[exercise of self-care agency(ESCA)],the negative emotion score[Hamilton anxiety scale-14(HAMA-14),Hamilton depression scale-17(HAMD-17)],the quality of life[general quality of life inventory(GQOLI-74)],and the nursing satisfaction were compared between the two groups before and after the nursing.Results:The 6 MWD,the step frequency,the ESCA score as well as the GQOLI-74 dimension scores and total score of the observation group after nursing were higher than those of the control group,and the differences were statistically significant(P<0.05).The HAMA-14 and HAMD-17 scores of the observation group after the nursing were lower than those of the control group,and the differences were statistically significant(P<0.05).Further,the each item score and the total score of nursing satisfaction in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusions:Teach-back health education combined with continuous nursing in the patients with cardiac valve replacement surgeries can improve the postoperative cardiac function,improve the self-care ability,alleviate the negative emotions,and improve the quality of life and the nursing satisfaction.
