中医综合方案干预中重度腰椎管狭窄症的新思路

摘　　要：退行性腰椎管狭窄症是一种老年多发疾病,在指南中,中重度退行性腰椎管狭窄症保守治疗被推荐为第一治疗策略,但没有给出具体高质量证据推荐的中医保守方案。本文以“以筋代骨”中医理论探求出中重度腰椎管狭窄症“杂合而治”中医保守诊疗方案的新思路。在刺络放血和药罐拔罐的基础上,结合针刀、膏摩、内服中药等构成综合疗法,以内服中药调理患者形神,以针刀增加治疗强度,以膏摩促进组织修复,形成“以罐为主、以刀助攻、以膏修复、以药调身”的四维一体疗法。Degenerative lumbar spinal stenosis is a disease frequently occurring in the elderly.Conservative treatment of moderate and severe degenerative lumbar spinal stenosis is recommended as the first treatment strategy in the guidelines,but there is no specific high-quality evidence to recommend conservative treatment of Chinese medicine.According to the Chinese medicine theory of“yi jin dai gu”as the treatment principle,a new idea of Chinese medicine comprehensive plan for intervention of moderate and severe lumbar spinal stenosis is explored,that is,a new idea of Chinese medicine conservative treatment plan of“heterozygosity and treatment”.On the basis of the pricking blood treatment and medicine cupping therapy,combined with acupotomology,ointment rubbing,Chinese medicine decoction and other treatment methods as one of the comprehensive therapy.Oral Chinese medicine is used to recuperate the patient’s body and mind,acupotomology is used to increase the intensity of treatment,and ointment rubbing is used to promote tissue repair,forming a four-dimensional integrated therapy of“bamboocuppingas the main therapy,acupotomology as the assist therapy,ointment rubbing as the repair therapy,and Chinese medicine decoction as to improve physical condition”.
