摘　　要：下肢深静脉血栓后综合征(PTS)是下肢深静脉血栓(DVT)形成后引起深静脉瓣膜破坏、血管闭塞所导致的继发性下肢慢性静脉疾病,其主要症状包括下肢肿胀、足靴区皮肤色素沉着及下肢慢性溃疡,严重影响患者的生活质量。目前,PTS的治疗方式包括保守治疗、外科手术和腔内治疗。近年来,随着对下肢深静脉PTS发病机制研究的不断深入以及腔内治疗技术、设备的不断改进,下肢深静脉PTS的治疗方式亦在不断优化。相较于保守治疗和外科手术,腔内治疗具有微创、高效等优势。腔内治疗的目的是开通阻塞的髂股静脉,降低静脉高压,缓解下肢症状,其治疗方式主要包括机械性血栓清除术、导管接触性溶栓(CDT)、髂静脉支架植入术等多种技术。本文就下肢深静脉PTS的腔内治疗进展进行综述,以期为临床工作的开展提供参考。Lower extremity deep vein post-thrombotic syndrome(PTS)is a secondary chronic venous disease of the lower extremity caused by deep vein valve destruction and vascular occlusion after deep vein thrombosis(DVT).The main symptoms of PTS include swelling of the lower extremity,skin pigmentation in the boot area and chronic ulcers of the lower extremity,which seriously affect the patients'quality of life.The current treatment of PTS includes conservative treatment,surgery and endovascular treatment.With the continuous in-depth research on the pathogenesis of lower extremity deep vein PTS in recent years and the improvement of endovascular treatment technology and equipment,the clinical treatment of lower extremity deep vein PTS is also continuously optimized.Compared with conservative and surgical treatment,endovascular treatment has significant advantages including minimally invasive and high efficiency.The purpose of endovascular treatment is to restore blocked iliac femoral veins,reduce venous hypertension,and relieve symptoms of lower extremity.The methods mainly include mechanical thrombectomy,catheter-directed thrombolysis(CDT),and iliac vein stent implantation.This article reviews the development of endovascular treatment of lower extremity deep vein PTS to provide reference for clinical practice.
