从郁、热、虚、损演变规律认识性早熟

摘　　要：性早熟的发展演变是一个动态过程。作为一种慢性代谢性疾病,性早熟的自然演变过程同样经历郁、热、虚、损4个阶段,分别代表疾病的发生、发展和结局。不同阶段病机及证候表现不同,如郁的阶段主要表现为痰湿壅滞证、肝郁气滞证,热的阶段主要表现为痰热互结证、肝郁化火证,虚的阶段主要表现为阴虚火旺证、脾虚肝郁证、上热下寒证等。不同阶段治疗原则亦有差别:郁的阶段以理气化痰、开郁清积为治疗根本,热的阶段以清热泻火为主,虚的阶段则虚实并重,滋阴疏肝和中、益肾养元壮骨为本。因此,对性早熟的论治应把握其全貌,明确其发展阶段,根据疾病的自身发展规律分阶段辨证论治。The development and evolution of sexual prematurity is a dynamic process.As a chronic metabolic disease,the natural evolution process of sexual prematurity also goes through four stages:depression,heat,deficiency and impairment,which represent the occurrence,development and outcome of the disease respectively.The pathogenesis and syndrome manifestations of sexual prematurity are different in different stages.For example,the stage of depression is mainly manifested as the syndrome of phlegm-dampness congestion and stagnation,and the syndrome of liver depression and qi stagnation,while the stage of heat is mainly manifested as the syndrome of interniingled phlegm and heat,and the syndrome of liver depression transforming into fire,yet the stage of deficiency is mainly manifested as the syndrome of hyperactivity of fire due to yin deficiency the syndrome of spleen deficiency and liver depression,the syndrome of upper heat and lower cold,etc.There are also differences in its treatment principles in different stages.For example,in the stage of depression,the treatment is based on regulating the qi-flowing for eliminating phlegm,and relieving depression and clearing accumulation,while in the stage of heat,the treatment is based on clearing heat-fire,and in the stage of deficiency,the treatment is based on dealing with both deficiency and excess,nourishing yin and dispersing stagnated liver qi and regulating the stomach,benefiting the kidney,nourishing yuan qi and strengthening bones.Therefore,the treatment of sexual prematurity should grasp its overall picture,define its development stages,and treat it on the basis of the syndrome differentiation in its different stages according to its own development laws.
