摘　　要：张文风教授认为,难治性偏头痛的病因病机与“阳化气、阴成形”功能失调有关,病机关键是病损后天致清阳不升,阳之气化不足而致阴浊内生,痰浊、瘀血阻滞脑络而成,临证有迁延不愈、反复发作的特点,治疗原则以扶正祛邪为主,予“升清阳、补元气、化痰浊、通经络、清虚热”等五种综合治疗方法,派方遣药时统筹全局、五法并用,予以“半夏白术天麻汤”加减治疗,攻补兼施、寒热并用、共同增效,为丰富难治性偏头痛的中医论治提供了思路与参考。Professor Zhang Wenfeng believes that the etiology of refractory migraine is related to the dysfunction of"yang transforming qi and yin shaping up the body",while the key to its pathogenesis is clear yang failing to ascend caused by the acquired disease damage,the accumulation of internal yin-turbidity caused by yang failing to transform qi,which leads to the blockage of brain channels by phlegm-turbidity and static blood.In clinic,intractable migraine features a prolonged course and repeated attacks.Accordingly,a priority should be given to strengthening the vital qi to eliminate pathogenic factors with regard to its treatment principle.Based on these insights,Professor Zhang Wenfeng creatively puts forward five composite treatment methods,including"ascending clear yang,invigorating primordial qi,resolving phlegm-turbidity,dredging meridians,and clearing deficient heat".She formulates prescriptions and decides on medicines in a holistic manner,taking into account all the five methods.She usually applies the Banxia Baizhu Tianma Decoction in the treatment of refractory migraine,with the dosages of ingredients being added or subtracted flexibly,in a bid to achieve reinforcement and elimination in combination,deal with cold and heat simultaneously,and strengthen the effectiveness.Her innovative practice provides a new idea to enrich the TCM treatment of intractable migraine.
