加味茯苓甘草汤治疗慢性阻塞性肺疾病缓解期

摘　　要：目的探讨加味茯苓甘草汤用于慢性阻塞性肺疾病(COPD)缓解期的价值。方法选择128例慢性阻塞性肺疾病患者分为观察组与对照组,各64例。对照组行氧疗、祛痰、运动康复训练、支气管扩张剂、抗感染药物等常规治疗,观察组在对照组基础上加服加味茯苓甘草汤。比较2组患者的治疗效果及治疗前后中医证候积分、T淋巴细胞(CD4^(+)、CD8^(+)及CD4^(+)/CD8^(+))、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT),最后分析其用药安全性。结果观察组总有效率(89.06%,57/64)显著高于对照组(75.00%,48/64)(P<0.05);观察组治疗后各项中医证候积分显著低于对照组及治疗前(P<0.05);观察组治疗后CD4^(+)、CD4^(+)/CD8^(+)水平显著高于对照组及治疗前,CD8^(+)水平显著低于对照组及治疗前(P<0.05);观察组治疗后PT、APTT、TT显著高于对照组及治疗前(P<0.05);2组用药期间均未见明显不良反应,且肝肾功能检查无明显异常。结论加味茯苓甘草汤用于慢性阻塞性肺疾病缓解期治疗能改善CD4^(+)、CD8^(+)及CD4^(+)/CD8^(+)水平,提高PT、APTT、TT,治疗效果明显。Objective To explore the value of modified Fuling Gancao Decoction for the chronic obstructive pulmonary disease(COPD)in its remission period.Methods From June 2018 to December 2020,a total of 128 patients with a chronic obstructive pulmonary disease were selected and divided into an observation group and a control group,with 64 patients in each group.The control group received a routine treatment such as oxygen therapy,expectorant,exercise rehabilitation training,bronchodilators,anti-infective drugs,etc.,while the observation group was additionally treated with the modified Fuling Gancao Decoction.The therapeutic effect,scores of TCM syndromes,T lymphocytes(CD4^(+),CD8^(+)and CD4^(+)/CD8^(+)),prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT)were compared between the two groups before and after treatment,and the drug safety was analyzed.Results The total effective rate of the observation group(89.06%,57/64)was significantly higher than that of the control group(75.00%,48/64)(P<0.05).The scores of TCM syndromes in the observation group after treatment were significantly lower than those in the control group and before treatment(P<0.05).The levels of CD4^(+)and CD4^(+)/CD8^(+)in the observation group after treatment were significantly higher than those in the control group and before treatment,while the level of CD8^(+)in the observation group was significantly lower than those in the control group and before treatment(P<0.05).PT,APTT and TT in the observation group after treatment were significantly higher than those in the control group and before treatment(P<0.05).There were no obvious adverse reactions during the treatment in both groups,and there were no obvious abnormalities in the liver and kidney function examinations.Conclusion The modified Fuling Gancao Decoction can improve the levels of CD4^(+),CD8^(+)and CD4^(+)/CD8^(+),improve PT,APTT and TT in the remission stage of chronic obstructive pulmonary disease.
