摘 要:目的 探讨小切口切开复位全螺纹空心螺钉内固定术治疗跟骨骨折的效果。方法 选择105例跟骨骨折患者为研究对象,采用随机数字表法将其分为试验组(n=53)和对照组(n=52)。对照组给予外侧L形切口钢板内固定术治疗,试验组给予小切口切开复位全螺纹空心螺钉内固定术治疗。比较两组患者的临床疗效、手术情况、跟骨结节关节角(Bohler角)、跟骨交叉角(Gissane角)、骨折愈合时间、足功能评分及并发症发生情况。结果 试验组患者的手术时间、术中出血量、住院时间、切口长度及术后引流量均少/短于对照组(均P<0.05)。术后6个月,试验组患者的临床疗效优于对照组,Gissane角大于对照组,骨折愈合时间短于对照组,足功能评分高于对照组,并发症总发生率低于对照组(均P<0.05)。结论 应用小切口切开复位全螺纹空心螺钉内固定术治疗跟骨骨折患者的疗效显著,可缩短骨折愈合时间,改善足功能,且并发症少。Objective To investigate the effect of mini-incision open reduction and full-thread cannulated screw internal fixation on calcaneous fracture. Methods A total of 105 patients with calcaneous fracture were selected as research objects and divided into experimental group(n=53) and control group(n=52) by the random number table method. The control group was treated with lateral L-shaped incision plate internal fixation, while the experimental group was treated with mini-incision open reduction and full-thread cannulated screw internal fixation. The clinical efficacy, operation condition, calcaneous tubercle joint angle(angle of Bohler), calcaneous intersection angle(Gissane angle), fracture healing time, foot function score and incidence of complications were compared between the two groups. Results The operation time, intraoperative blood loss, hospital stay, incision length and postoperative drainage volume of experimental group were all less/shorter than those of control group(all P<0.05). At the 6 th month after operation, the clinical efficacy of the experimental group was better than that of the control group, the Gissane angle was larger than that of the control group, fracture healing time was shorter than that of the control group, foot function score was higher than that of the control group, and the total incidence of complications was lower than that of the control group(all P<0.05). Conclusion The application of mini-incision open reduction and full-thread cannulated screw internal fixation has significant effect on patients with calcaneous fracture, which can shorten fracture healing time and improve the foot function with few complications.
