地榆汤加减联合美沙拉嗪对湿热困阻型溃疡性结肠炎患者的临床疗效

摘　　要：目的探讨地榆汤加减联合美沙拉嗪对湿热困阻型溃疡性结肠炎患者的临床疗效。方法124例患者随机分为对照组和观察组,每组62例,对照组给予美沙拉嗪,观察组在对照组基础上加用地榆汤加减,疗程4周。检测临床疗效、IL-6、IL-10、TGF-β1、CD4^(+)、CD4^(+)/CD8^(+)、肠道菌群(乳酸杆菌、肠球菌、双歧杆菌)、SF-36评分、中医证候评分、Mayo评分变化。结果观察组总有效率高于对照组(P<0.05)。治疗后,2组IL-6、CD4^(+)、CD4^(+)/CD8^(+)、Mayo评分、中医证候评分降低(P<0.05),TGF-β1、IL-10、SF-36评分升高(P<0.05),乳酸杆菌、双歧杆菌增加(P<0.05),肠球菌减少(P<0.05),以观察组更明显(P<0.05)。结论地榆汤加减联合美沙拉嗪可减轻湿热困阻型溃疡性结肠炎患者炎症反应,调节肠道菌群,促进免疫功能恢复,提高临床疗效。AIM To explore the clinical effects of Modified Diyu Decoction combined with mesalazine on patients with ulcerative colitis of Damp-heat Impediment Pattern.METHODS One hundred and twenty-four patients were randomly assigned into control group(62 cases)for 4-week administration of mesalazine,and observation group(62 cases)for 4-week administration of both Modified Diyu Decoction and mesalazine.The changes in clinical efficacy,IL-6,IL-10,TGF-β1,CD4^(+),CD4^(+)/CD8^(+),intestinal flora(Lactobacillus,Enterococcus,Bifidobacterium),SF-36 score,TCM syndrome score and Mayo score were detected.RESULTS The observation group demonstrated higher total effective rate than the control group(P<0.05).After the treatment,the two groups displayed decreased IL-6,CD4^(+),CD4^(+)/CD8^(+),Mayo score,TCM syndrome score(P<0.05),increased TGF-β1,IL-10,SF-36 score(P<0.05),elevated Lactobacillus,Bifidobacterium(P<0.05),and reduced Enterococcus(P<0.05),especially for the observation group(P<0.05).CONCLUSION For the patients with ulcerative colitis of Damp-heat Impediment Pattern,Modified Diyu Decoction combined with mesalazine can alleviate inflammatory responses,regulate intestinal flora,promote immune function recovery,and improve clinical efficacy.
