陈士铎辨治癃闭思想探析

摘　　要：通过阅读清代名医陈士铎《辨证录》中有关癃闭的内容,采用归纳整理、总结分析的研究方法,分别从以下几个方面展开论述:癃闭的虚实性质及发病病位分类、病理机制探讨、治疗原则等进行总结,从而反映陈士铎辨治该病的学术思想。陈士铎认为癃闭的主要发病部位在膀胱,病机总归于膀胱气化不利,分析了心火亢盛、热结膀胱、阴阳失常、肺燥津伤、脾胃气虚等证型所致癃闭的病机特点,并确立了相应的治疗方法。陈士铎对癃闭辨证分型精准深入,遣方用药灵活有效,每则病案都有其鲜明特点,这对于进一步理解癃闭具有重要意义,也为中医临床提供了一些具体可行的治疗思路。By reading the contents of the famous doctor Chen Shiduo's Syndrome Differentiation Recordin Qing Dynasty,and adopting the research methods of induction,collation and summary analysis,this paper discusses from the following aspects:the nature of deficiency and excess,the classification of disease location,the discussion of pathological mechanism,and the summary of treatment principles,thus reflecting Chen Shiduo's academic thought of syndrome differentiation and treatment.It is believed that the main site of Chen's claustrophobia is bladder,and the pathogenesis is always due to unfavorable bladder gasification.This paper analyzes the pathogenesis characteristics of claustrophobia caused by excessive heart fire,heat accumulation in bladder,yin-yang disorder,lung dryness and fluid injury,spleenstomach qi deficiency and other syndromes,and establishes the corresponding treatment methods.Chen Shiduo's accurate and in-depth syndrome differentiation and classification of lumbago,flexible and effective prescription and medication,each case has its own distinctive features,which is of great significance for further understanding of the disease of lumbago and provides some concrete and feasible treatment ideas for TCM clinic.
