改良注水法在老年患者清醒结肠镜检查中的应用研究

摘　　要：目的观察改良注水法在老年患者清醒结肠镜检查中的应用效果。方法选取2020年1月—2021年6月在济宁市兖州区人民医院进行清醒结肠镜检查的老年患者110例为研究对象,采用随机数表法将其1:1分为对照组和试验组。对照组采用常规注气法检查,试验组采用改良注水法检查,比较检查结果、疼痛度和舒适度及不良反应情况。结果两组患者的镜检达盲率、不良反应率相近,差异无统计学意义(P>0.05)。相较于对照组,试验组病灶检出率更高(74.55%vs.52.73%),达盲和退镜时间更短,差异有统计学意义(P<0.05)。试验组镜检中,辅助操作使用率更低(5.45%vs.18.18%),差异有统计学意义(P<0.05)。试验组在检查中和检查后的疼痛数字评分(numerical rating scale,NRS)更低、舒适状况评分(general comfort questionnaire,GCQ)更高,差异有统计学意义(P<0.05)。结论老年患者清醒结肠镜检查中采用改良注水法,能优化进镜操作,提高病灶检出率和患者的舒适程度,为结肠镜检查肠道准备指南的制定和优化提供借鉴,值得推广。Objective To observe the effect of improved water injection method in conscious colonoscopy in elderly patients.Methods A total of 110 elderly patients who underwent conscious colonoscopy in Yanzhou District People’s Hospital from January 2020to June 2021 were selected.Using random number table method,they were divided into control group and experimental group 1:1.The control group was examined by routine air injection method,and the experimental group was examined by improved water injection method.The examination results,pain,comfort and adverse reactions were compared.Results The blind rate and adverse reaction rate of the two groups were similar,and there was no significant difference(P>0.05).Compared with the control group,the detection rate of lesions in the experimental group was higher(74.55%vs.52.73%),and the time of blindness and withdrawal was,the differences were statistically significant(P<0.05).In the test group,the use rate of auxiliary operation was lower(5.45%vs.18.18%),with statistical difference(P<0.05).The experimental group had lower numerical rating scale(NRS)and higher general comfort questionnaire(GCQ)during and after the,the differences were statistically significant(P<0.05).Conclusion In the conscious colonoscopy of elderly patients,the improved water injection method can optimize the endoscopic operation,improve the detection rate of lesions and the comfort of patients,which is worthy of popularization.
