后路短节段固定结合不同植骨方式治疗轻、中度胸腰椎爆裂性骨折的疗效分析

摘　　要：目的:分析后路短节段固定结合不同植骨方式对轻、中度胸腰椎爆裂性骨折的疗效.方法:选取本院2017年2月至2020年8月收治的72例轻、中度胸腰椎爆裂性骨折患者,根据不同手术方式分为研究组和对照组,每组各36例.研究组行后路短节段固定+经伤椎椎弓根椎体内植骨;对照组行后路短节段固定+外侧植骨融合.比较两组的手术效果.结果:研究组手术时间、术中出血量、伤椎后凸Cobb角、Oswestry功能障碍指数(ODI)评分及不良反应发生率均低于对照组,且伤椎前缘高度矫正率更高(P<0.05).结论:轻、中度胸腰椎爆裂性骨折患者采用后路短节段固定术+经伤椎椎弓根椎体内植骨术的创伤性小,疗效优于后路短节段固定结合外侧植骨融合术.Objective:To analyze the clinical effect of posterior short segment fixation combined with different bone grafting methods in patients with mild to moderate thoracolumbar burst fractures.Methods:Seventy-two patients with mild to moderate thoracolumbar burst fractures admitted to our hospital from February 2017 to August 2020 were divided into study group and control group according to different surgical treatment methods,with 36 cases in each group.The study group underwent posterior short segment fixation and transpedicular bone grafting on the injured vertebrae,and the control group was treated with posterior short segment fixation and posterolateral bone grafting and fusion.The clinical effects were compared between the two groups.Results:The operation time,intraoperative blood loss,Cobb angle,the Oswestry disability index(ODI)score and the incidence of adverse reactions in the study group were lower than those in the control group,and the correction rate of the height of the anterior edge of the injured vertebra was higher in the study group(P<O.05).Conclusion:In patients with mild to moderate thoracolumbar burst fractures,posterior short segment fixation combined with transpedicular bone grafting on the injured vertebrae is less traumatic,and the curative effect is better than posterior short segment fixation combined with posterolateral bone grafting and fusion.
