传统高位结扎加分段剥脱术、静脉腔内激光术与透光旋切术治疗大隐静脉曲张患者的疗效比较

摘　　要：目的:比较传统高位结扎加分段剥脱术、静脉腔内激光术与透光旋切术治疗大隐静脉曲张患者的疗效.方法:回顾性分析2010年1月至2020年1月本院收治的412例大隐静脉曲张患者的临床资料,其中行传统高位结扎加分段剥脱术的患者为A组,共137例;行静脉腔内激光术的患者为B组,共134例;行透光旋切术的患者为C组,共141例.对比分析三组患者的手术一般情况及术后并发症发生情况.结果:B、C组患者的手术时间、手术出血量及住院时间显著短于、低于A组,但C组患者的手术费用明显高于A、B组(P<0.05).B、C组患者术后并发症发生率明显低于A组(P<0.05);A组患者的复发率显著高于B、C组(P<0.05).结论:3种手术方法在治疗大隐静脉曲张患者上均有其各自的适应证,因此临床医师应根据患者的实际情况及意愿选择合适的手术方法.Objective:To compare and analyze the efficacy of traditional high ligation segmentation stripping operation,endovenous laser treatment and transilluminated powered phlebectomy in the treatment of great saphenous varicose veins.Methods:The clinical data of 412 patients with great saphenous varicose vein in our hospital from January 2010 to January 2020 were analyzed retrospectively.In group A,137 patients were treated by traditional high ligation segmentation stripping operation.In group B,134 patients were treated by endovenous laser treatment.In group C,141 patients treated by transilluminated powered phlebectomy.The general surgery and postoperative complications of the three groups were compared and analyzed.Results:The operative time,operative blood loss and hospital stay in group B and C were significantly lower than those in group A,but the operative cost in group C was significantly higher than that in group A and B(P<0.05).The incidence of postoperative complications in group B and C was significantly lower than that in group A(P<0.05).The recurrence rate in group A was significantly higher than that in group B and C(P<0.05).Conclusion:The three surgical methods have their own indications in the treatment of patients with great saphenous varicose veins,so clinicians should choose the appropriate surgical methods according to the actual situation and desire of patients.
