瘢痕子宫再次妊娠孕妇发生凶险性前置胎盘的影响因素分析

摘　　要：目的:探讨瘢痕子宫再次妊娠患者凶险性前置胎盘(PPP)发生情况及其影响因素.方法:选取2019年1月至2021年1月在本院产检并分娩的73例瘢痕子宫再次妊娠患者,统计患者PPP发生情况,设计临床资料调查问卷,分析瘢痕子宫再次妊娠患者并发凶险性PPP的影响因素.结果:经评估,73例瘢痕子宫再次妊娠患者中发生凶险性PPP者22例,占比30.14%;发生组距离上次剖宫产时间短于未发生组,有人工终止妊娠史和有子宫内膜炎占比高于未发生组(P<0.05);经Logistic回归分析显示,距离上次剖宫产时间长是瘢痕子宫再次妊娠患者PPP发生的保护因素(OR<1,P<0.05);有人工终止妊娠史、有子宫内膜炎是瘢痕子宫再次妊娠患者PPP发生的危险因素(OR>1,P<0.05).结论:距离上次剖宫产时间长是瘢痕子宫再次妊娠患者PPP发生的保护因素,有人工终止妊娠史、有子宫内膜炎是瘢痕子宫再次妊娠患者PPP发生的危险因素.Objective:To investigate the occurrence and influencing factors of pernicious placenta previa(PPP)in re-pregnant patients with scar uterus.Methods:Seventy-three cases of re-pregnant patients with scar uterus who were examined and delivered in Luoshan County Maternal and Child Health Care Hospital from January 2019 to January 2021 were selected.The occurrence of PPP in patients was counted and designed the clinical information questionnaire.The influencing factors of PPP in repregnant patients with scar uterus were found out.Results:After evaluation,among 73 repregnant patients with scar uterus,22 cases had PPP,accounting for 30.14%;the time from the last cesarean section in the occurrence group was shorter than that in the non occurrence group,and the proportions of induced abortion and endometritis were higher than those in the non occurrence group(P<0.05);through Logistic regression analysis,it was showed that the long time from the last cesarean section was the protective factor of PPP in re-pregnant patients with scar uterus(OR<1,P<0.05);the history of induced abortion and endometritis were the risk factors of PPP in re-pregnant patients with scar uterus(OR>1,P<0.05).Conclusion:The long time from the last cesarean section is a protective factor for PPP in re-pregnant patients with scar uterus,the history of induced abortion and endometritis are the risk factors of PPP in re-pregnant patients with scaruterus.
